Giant brain arteriovenous malformation managed by staged embolizations and surgical resection with satisfactory outcome.

World Neurosurgery 2023 September 24
Spetzler-Martin (SM) grade V (>6 cm) arteriovenous malformation (AVM) are traditionally considered inoperable. A 35-year-old male presented with repeated seizures for 7 years and computed tomography arteriography and magnetic resonance imaging revealed left deep hemispheric AVM. A combination of embolization and surgical resection successfully achieved a cure of the patient. Well-equipped neurosurgery facilities can best manage selective SM grade V AVMs with no neurological deficits contrary to their traditionally inoperable concept. Successful surgery offers the patient a better quality of life.
