OBJECTIVE: This paper describes my experience as a psychiatric registrar working with indigenous communities in a remote setting in Far North Queensland. CONCLUSIONS: Working in a remote environment provided an insight into how to engage culturally diverse Indigenous communities and an invaluable training experience.

INTRODUCTION: Endovascular repair of aortic aneurysm (EVAR) has a lower mortality than open repair. The aim of this study was to assess mortality from EVAR for emergency AAA repair and the impact of fitness for operation and adverse anatomy. METHODS: One-hundred and forty two patients who had EVAR for a ruptured AAA (80, REVAR) or a symptomatic AAA (62, SEVAR) between 1994 and 2007 in a single specialist endovascular centre were reviewed. Fitness for surgery was assessed by Hardman's index (age>76, loss of consciousness, Hb<9...

Using the principle of quasi-continuous filtering in a non-linear fibre, we propose an optical device for the simultaneous regeneration of sevaral channels at 40 Gbit/s. Simulations predict an improvement of the signal quality for four channels by more than 6.8 dB.

Four cases treated with Aurum muriaticum natronatum are presented using the patients' own words; two cases of fibromyoma of uterus and thyroid disorders, and two of eczema and depression with suicidal ideation after disappointed romantic love. The case analyses and management are discussed and distilled into the materia medica of Aurum muriaticum natronatum with the creation of a heuristic.

BACKGROUND: The Tibetan Delek Hospital is a small general hospital providing primary and secondary care for the Tibetan refugee community and the local Indian population in Dharamsala, Himachal Pradesh, North India. In a baseline clinical audit of intrapartum care at the Tibetan Delek Hospital in 1996, high levels of postpartum haemorrhage associated with poor medical management of the third stage of labour, plus inappropriate transfer of women in labour were observed. These audit findings prompted the implementation of changes in the delivery of intrapartum care and follow-up audit cycles to monitor the ongoing effect of these changes...

This paper reports an audit of clinical outcome in 455 consecutive patients (1100 consultations) presenting for private homeopathic treatment of a chronic illness in which conventional treatment had either: failed, reached a plateau in effect, or was contra-indicated by side effects, age or condition of the patient. Three hundred and four patients (66.8%) derived benefit from homeopathic treatment. One hundred and forty-eight patients (32.5%) were able to stop or maintain a substantial reduction in their conventional drugs...

The cases presented are of three children who responded well to Sanicula aqua. I have used the exact words of the patients or their mothers. All 3 are: firstborn girls, tall and very thin with blue-green eyes. Two with recurrent otitis media with perforated tympanic membrane and smell of rotting fish, the third atopic eczema and asthma. The case analyses, materia medica of Sanicula aqua and the creation of a remedy heuristic, are discussed.

An audit was conducted of 829 consecutive patients presenting for homeopathic treatment of a chronic illness, conventional treatment had either failed, plateaued in effect, or was contraindicated by adverse effects, age or condition of the patient. Of the 829 patients, 503 (61%) had a sustained improvement from homeopathic treatment, of these: * 357 patients (43%) had an excellent response; * 146 patients (18%) had a good response; * 6 patients (0.8%) became worse. * 233 patients (28%) were lost to follow-up...

OBJECTIVES: An experimental model of Chlamydia trachomatis salpingitis was developed in mice to study the effect of combination antibiotic and antiinflammatory therapy on restoring fertility after tubal infection. METHODS: The mice were infected by injection via the ovarian bursa with a suspension containing a human strain of Chlamydia trachomatis sevar F. After treatment with antibiotics alone or in combination with antiinflammatory agents, fertility was studied at 4 months...

COMPARATIVE STUDY
A H Sevars, R B Schasfoort, M H Salden
In this paper the development of a surface plasmon resonance (SPR) immunosensor for syphilis screening is described. This immunosensor is based on the detection of antibodies in serum against the causative organism Treponema pallidum. In order to achieve selectivity a recombinant Treponema pallidum membrane protein A (TmpA) was used. This antigen can react with antibodies to T. pallidum, present in serum of syphilitic patients. Reproducible results have been obtained, using a 'sandwich SPR' method: binding of a sandwich antibody to the treponemal antibody after serum incubation was measured in real time while the binding was taking place...
