Regenerative therapy of peri-implantitis - Clinical and radiological documentation of 16 consecutive patients with a mean follow-up of three years.

Sixteen patients with a diagnosis of peri-implantitis in 19 of 64 implants (29.7 %) were treated with an open flap approach according to a refined standardized surgical protocol. Treatment consisted of mechanical debridement, application of chemotherapeutical agents, and photodynamic therapy. Bone defects were filled with autologous and allogeneic bone, and subsequently covered with a collagen membrane and a Platelet rich in growth factors fibrin membrane. After a mean follow-up of 36.1 months, none of the treated implants was lost. Mean bleeding of probing was reduced from 34.84 % to 14.79 %. Mean plaque index decreased from 41.89 % to 20.00 %. Probing depth and recession defects decreased from 6.48 mm to 3.25 mm, and 1.79 mm to 0.11 mm, respectively. Mean gain of crestal bone level was 3.78 mm. All post-operative improvements of clinical and radiological parameters were statistically significant. Within the limitations of this case series, the introduced treatment approach may have a beneficial impact as a standardized peri-implantitis-therapy protocol.
