Self-assessment toolkit.

A new health and integration toolkit has been launched by NHS Clinical Commissioners, in partnership with the Local Government Association, NHS Confederation and the Association of Directors of Adult Services. The self-assessment tool is designed to help local health and care leaders, through health and well-being boards, to assess their ambition, capability, capacity and readiness to integrate local health and social care services.
