Caused by the pressure of reduction of expenses there is the question of the economic benefit if done outpatient in Germany. A comparison of the inpatient and outpatient gratification will be done. There is a yearly potential of cost reduction of 180 million euro, if the operation would be done as an outpatient procedure. The following article presents nasal septum and turbinate surgery. First an overview with special consideration of the anatomical and physiological background is given followed by indications for surgical procedures. Endonasal sinus surgery in the case of inflammatory disease is of increasing importance. The maxillary, ethmoid, sphenoid and frontal sinus operations as well as the septumplasty are performed microsurgically. There are only few indications, when the intranasal procedure cannot be applied. We prefer all intranasal operations with the patient placed in semi-Fowler's position with hypotensive anaesthesia. The histological findings of the crushed deviated septal cartilage and bone are described directly after reimplantation between the mucoperichondrium periostium in septoplasty, and after 11, 16, 24 and 26 months as well as after 6 and 6.5 years. Immediately after crushing, multiple fractures different in width and damage of single cartilage cells and bone cells are visible. The crushed reimplanted material shows partial absorption and partial regeneration especially near the fracture lines over a period of several years...
