We report a case of primary diffuse leptomeningeal gliomatosis (PDLG) in a 76-year-old male presenting with confusion, dysarthria, diplopia, lumbal pain and headaches of recent onset. Neurological examination revealed nuchal rigidity and bilateral sixth cranial nerve palsy. The cerebrospinal fluid showed a marked hyperproteinorachia (4711 mg/L) and mild cytorachia (5-10 leucocytes/mm3) with a few atypical lymphoid cells. On admission, brain CT scan and MRI demonstrated diffuse and nodular leptomeningeal contrast enhancement predominant at the skull base and several osteolytic lesions in the right parietal bone...

The aim of our retrospective study is to show how very circumspect we are to use this method. The clinical symptoms of our patients were lumbalgia or lumboischialgia with or without neurological deficiencies. All these patients were resistent to conservative therapies. We accepted for operation only patients who suffered from protrusions and types of prolaps without perforation of the posterior longitudinal ligament. 179 operations on the vertebral discs were performed in our department between 01. 01. 1992 until 31...

A two-year-old patient was diagnosed with arthrogryposis because of inflected knees (15-20 degrees) and stiff hamstrings. All joints and reflexes were normal. Activities were described as age-appropriate, but the patient walked with inflected knees and increased lumbal lordosis. Physiotherapy and botox did not improve the condition. Because of pain and hampered activities, the patient - now aged 11 years - was treated with musculoskeletal medicine. After treatment the patient could walk normally without pain and stand with stretched knees...

A 50-year-old man presented to the out-patient clinic because of a sudden pain in his left eye just after lumbal magnetic resonance imaging (MRI) was started. Biomicroscopic examination of the left eye revealed the inferior anterior chamber angle including a foreign body and the superior anterior chamber angle including the iris portion, into which the foreign body had been stuck prior to the MRI examination. His history revealed an accidental penetrating eye trauma of the shotgun pellet of 28 years ago. The foreign body was easily removed using a forceps intraoperatively...

Pure spinal epidural cavernous angiomas are extremely rare lesions, and their normal shape is that of a fusiform mass in the dorsal aspects of the spinal canal. We report a case of a lumbo-sacral epidural cavernous vascular malformation presenting with acute onset of right-sided S1 radiculopathy. Clinical aspects, imaging, intraoperative findings, and histology are demonstrated. The patient, a 27-year-old man presented with acute onset of pain, paraesthesia, and numbness within the right leg corresponding to the S1 segment...

There is a growing evidence that chemokines and their receptors play a role in inducing and maintaining neuropathic pain. In the present study, unilateral chronic constriction injury (CCI) of rat sciatic nerve under aseptic conditions was used to investigate changes for stromal derived factor-1 (SDF1) and its CXCR4 receptor in lumbal (L4-L5) and cervical (C7-C8) dorsal root ganglia (DRG) from both sides of naïve, CCI-operated and sham-operated rats. All CCI-operated rats displayed mechanical allodynia and thermal hyperalgesia in hind paws ipsilateral to CCI, but forepaws exhibited only temporal changes of sensitivity not correlated with alterations in SDF1 and CXCR4 proteins...

PURPOSE: In male patients the pudendal block has been applied only in rare cases as a therapy for neuralgia of the pudendal nerve. Up to now there is no comparison of pudendal block with an anaesthesia form close to the spinal cord. In this pilot study the bilateral perineal infiltration of the pudendal nerve was compared to combined spinal-epidural anaesthesia in high-dose-rate (HDR) brachytherapy. MATERIAL AND METHODS: In 30 patients (68.8 +/- 5.4 years) who underwent a core needle biopsy for high-risk prostate carcinoma, an HDR brachytherapy in CSE or NPB was carried out...

Local intracellular signaling cascades following peripheral nerve injury lead to robust axon regeneration and neuropathic pain induction. Cytokines are classic injury-induced mediators. We used sciatic nerve ligature (ScNL) to investigate temporal changes in IL-6 and its receptor gp130 in both ipsilateral and contralateral lumbal (L4-L5) dorsal root ganglia (DRG). Rats were operated aseptically on unilateral ScNL and allowed to survive for 1, 3, 7, and 14 days. Immunohistochemistry and Western blot analysis were used to determine levels of IL-6 and gp130 in DRG...

Dentists, dental hygienists and dental assistants due to the nature of their work-related physical activity often have to deal with physical stress that will cause actual physical damage. The aim of the current survey was to analyse the frequency of job-related pain and associated impairment. The questionnaire for the cervical and lumbal region of the North American Spine Society was modified and used in the present investigation. Out of 6962 subjects, 2025 returned the questionnaire (response rate 34.5%). Between 20% and 36% of the participants suffered from pain in the cervical and lumbal region and were impaired in their daily activities, particularly during the lifting of objects, while standing or sleeping...

AIM: The use of a spine orthosis for the treatment of low back pain is an approved, but also a disputed therapy method. There have been numerous studies so far that attempt to examine the acceptance and the impact of this treatment method. But up to now there is no biomechanical assessment about the effective level of intervertebral disc decompression while wearing an orthosis. Therefore, the purpose of this study is to measure the reduction of the disc pressure while wearing an orthosis...

A MRI of the lumbal spine was performed on a patient with lumbago. Besides the fact of no evidence for a myelopathy caused by a herniated disc a both-sided iliac vein thrombosis could be seen. In further examination the coincidence of a congenital absence of vena cava inferior with distinctive collateral vessel systems over lumbal and spinal veins and heterozygosis for Factor V Leiden abnormality could be identified as cause for thrombosis. It is very likely that symptoms developed as a result of spinal vein hyperplasia and consecutive myelon compression...

BACKGROUND: Myotonic dystrophy Curschmann-Steinert is a multisystemic disorder inherited as an autosomal dominant trait. Characteristic clinical findings are muscular weakness and distal atrophy of the extremities, myotonia and myopathic facies. Ophthalmological symptoms may include cataract, ptosis and intraocular hypotension. Cardiac conduction defects, endocrine disturbances, respiratory insufficiency and gastrointestinal affection appear quite often in the course of the disease. Because of the great diversity of symptoms and the variable clinical presentation the disease may be overlooked...
