The management of tachycardias depends on their underlying pathophysiology. The key to uncovering this pathophysiology is in finding the temporal relationship between atrial and ventricular activation. The P-waves resulting from atrial activation can however be hard to detect on a traditional EKG in the setting of a tachycardia. Esophageal-EKG can help reveal the P-waves. The patient swallows an electrode, whose position in the esophagus is then adjusted to maximize the signal coming from the left atrium, clearly revealing atrial activity...

Gastroesophageal reflux disease (GERD) is characterized by regurgitation of gastric juices into the esophagus. This has an erosive effect on the mucosa with accompanying symptoms, such as heartburn, acid regurgitation and positional-/exertion--induced chest pain. The associated inflammation in the multi-layered squamous epithelium of the esophagus (esophagitis) can usually be seen macroscopically at gastroscopy and is always possible to demonstrate microscopically as well-characterized changes. GERD is abundant in the adult population in the Western world, and the incidence appears to be increasing...

Nuclear imaging has used initially anatomic and volumetric technologies as CT or MRI. In recent years new dimensions of non invasive studies, as PET, have shown a higher utility in the effectiveness of the treatment. The evaluation of need must be done according to a principle of Horizontal Equity, equal treatment for equal need and of a principle of Vertical Equity, Different treatment, at regional level, according to each hospital level. The evaluation of need has been made according to the Potential Demand by Potential User Groups: diabetes, type 2, (50 years and more); screening colorectal (50 years and more); morbidity by cancer; surgery of lung cancer; cardiology; heart surgery; acute chest pain in the emergency department...

COMPARATIVE STUDY
M W Ingeman-Nielsen
X-ray examinations of the stomach and duodenum from a two year period of 135 Greenlanders and an equivalent number of Danes were reviewed in order to diagnose all radiological lesions. The results are presented. In 37% of the Greenlanders and 51% of the Danes, x-rays showed normal findings. Peptic ulcerations were diagnosed for the first time in 2.4% of Greenlanders over the age of 15 years. The incidence of gastric ulcer was 1.5% and of duodenal ulcer 0.9%. The male/female ratio was 1.3 in Greenlanders. The esofagus and cardia region of the Greenlanders revealed many serious lesions, emphasizing the importance of further investigations in this area...
