Dabigatrane is the first new generation antiacoagulant that at a dose of 150 mg twice daily was, in the RE-LY study, more effective in cerebrovascular event and systemic embolisation prevention than well-controlled warfarin and had comparable or better safety profile than well-controlled warfarin. The 110 mg twice daily dose showed comparable efficacy and superior safety against warfarin. Crucial and unique among modern anticoagulants is its ability to significantly reduce ischemic as well as hemorrhagic CVEs at a dose of 150 mg twice daily...

OBJECTIVES: Dabigatran etexilate is a new oral direct thrombin inhibitor for prophylaxis of venous thromboembolism (VTE) in patients who have elective surgery for total hip replacement (THR) or total knee replacement (TKR). Among the advantages of dabigatran etexilate over subcutaneous prophylaxis with Low Molecular Weight Heparin (LMWH) are reduced resource uses for (i) teaching patients to self-inject; (ii) home-care visits for subcutaneous administration; and (iii) absence of heparin-induced thrombocytopenia (HIT)...

BACKGROUND: The risk of venous tromboembolism (VTE) in women taking combined oral contraceptives (COCs) is attributed to changes in coagulation and fibrinolysis. The impact of the COCs may be greater in women with preexisting thrombophilic defects. Nevertheless most women who suffer from venous thrombosis do not have any of the well known hereditary or acquired risk factors. A simple and sensitive marker of"thrombogenicity" has not been identified. OBJECTIVES: To investigate the effects of two different monophasic combined oral contraceptives (COCs) on the plasma concentrations of activated protein C-inhibitor of protein C ( APC-PCI) and on comparable hemostatic factors in fertile women...

In malignant gliomas, the management of symptoms and minimization of side effects assume major importance. Corticosteroids provide transient relief from neurological symptoms. However, treatment with steroids is also commonly associated with considerable side-effects including: hyperglycemia, osteoporosis, myopathy, lymphopenia and others. Sometimes, antiepileptic drugs may contribute to clinical decline of neuro-oncological patients in stable disease not only by neuropsychological impairment but also by metabolic interations...

Oral contraceptives increase the risk of thromboembolism. Recent studies have indicated that the risk of thromboembolic disease in users of combined oral contraceptive pills varies not only with estrogen dose, but also with the type of gestagen in pills with the same estrogen dose. We describe 8 women from our intensive care unit who developed a deep venous thrombosis while on oral contraceptives. The aim of this article is also to discuss other risk factors for the potential risk of venous tromboembolism in users of combined oral contraceptive...

Total hip replacement represents an operation with high risk of thromboembolic disease. An optimal strategy of thromboprophylaxis is very important. In our two groups of patients we compared two ways of prophylaxis both during hospitalization and after hospital discharge. 81 patients in the first group were treated with low molecular weight heparin (LMWH) - Fraxiparine during the stay in hospital and with a combination of three drugs - Aspirin, Curantyl and Secatoxin during 3 months after operation. 85 patients in the second group were also treated with LMWH during the stay in hospital but LMWH was early changed for Warfarin...

INTRODUCTION: Hemostasis is a very important mechanism, whose changes can cause different complications. In the course of surgical interventions some changes in the system of coagulation happen. Laparoscopic cholecystectomy is a method of choice in the treatment of gallbladder calculosis. In the course of the procedure, parameters of hemostasis change, which stimulates a possible appearance of thromboembolic complications. The objective of our research was to reveal the changes in the system of coagulation in patients treated by laparoscopic cholecystectomy...

UNLABELLED: Fusiform and dolichoectatic aneurysms are challenging lesions to treat with direct clipping. Treatment of these aneurysms often requires alternative surgical strategies, including extracranial-intracranial bypass, wrapping, or clip-wrap techniques. Nonetheless, these alternatives methods of treatment have been underused and frequently overlooked. OBJECTIVE: To report a series of nine cases of otherwise untreatable aneurysms managed using the clip-wrap technique and discuss its surgical nuances...

Aromatase inhibitors are now the first choice endocrine therapy in the metastatic setting for postmenopausal women. These endocrine agents also seem likely to soon become the standard adjuvant therapy for postmenopausal patients with hormone-responsive breast cancer, either alone or in sequence with tamoxifen. In the treatment with aromatase inhibitors, the incidence of some adverse effects such as endometrial cancer, stroke or pulmonary tromboembolism treatment associated with tamoxifen is reduced. Questions remain about the long-term side-effects and safety profile of aromatase inhibitors--monitoring and handling of bone loss associated with their application are essential and are being addressed in ongoing trials...

The Authors present a case of a 11 year-old patient with a history of Juvenile Polyposis Syndrome (JPS), a condition characterized by the occurrence of multiple hamartomatous polyps in the gastrointestinal tract. Patients with JPS are traditionally treated by repeated endoscopic polypectomies and elective surgery. Recent studies reported up-regulation of cyclo-ossigenase 2 (COX-2) in colorectal polyps. Specific COX-2 inhibitors have been withdrown from the market for tromboembolic side effects. However efficacy and safety of preferential selective COX-2 inhibitor has been reported as antiinflammatory drugs also in children...

Antiphospholipid syndrome (Hughes syndrome) is an autoimmune disorder with production of antibodies against phospholipids and phospholipid binding peptides in human organism, that is associated with thrombotic events. Venous and/or arterial tromboembolism and recurrent pregnancy loss at women are the most frequently observed clinical manifestations. Neurological symptoms are common and they can often mime multiple sclerosis. That is why antiphospholipid antibodies must be concerned in differential diagnostics of multiple sclerosis, particularly with the atypical course and clinical pattern...

Cerebral arteriovenous malformation (AVM) is a complex network of vascular channels consisting of arterial feeders, a nidus and enlarged venous drainage. AVMs usually increase in size with time, but may rarely obliterate; spontaneous angiographic regression occurs in less than 1.5% of cerebral AVMs. Several causes of spontaneous regression have been postulated such us hemodynamic alterations due to hemorrhage, hypercoagulability, atherosclerosis, and tromboembolism from associated aneurysms. In this report we describe a case of spontaneous, complete and asymptomatic occlusion of a left cerebellar hemispheric AVM; angiograms clearly demonstrate a progressive decrease in size of the AVM at follow-up...
