Iniencephaly is a rare form of neural tube defect with an incidence of 0.1-10 in 10,000 pregnancies. It is characterized by the presence of occipital bone defects at foramen magnum, fixed retroflexion of head, spinal dysmorphism, and lordosis of cervicothoracic vertebrae. It is usually associated with central nervous system, gastrointestinal, and cardiovascular anomalies. PURPOSE: To determine the clinical importance and origin of a bifid configuration of the fetal sacrum seen during routine fetal sonography. MATERIALS AND METHODS: An apparent bifid malformation of the sacral spine was seen in 24 fetuses at antenatal sonography. An attempt was made to recreate this configuration prospectively in 111 consecutive second- and third-trimester fetuses. Three cadavers were also imaged to determine the origin of this configuration. An evaluation was made of all cases of neural tube defects born in seven gynaecological hospitals and obstetric centres of Havana, between 1970 and 1980. Neural tube defects were recordable from 492 infants out of 319,228 births. The families concerned were briefed in human genetics and informed about the possibility of prenatal diagnosis of anencephaly and bifid spine (spina bifida aperta). Some of them had a record of this defect. Amniocentesis was applied to 30 women with an empirical risk of 1:20, and alphafoetoprotein was analysed in their amniotic fluid...

Eight cases with the VATER association of malformations are presented. Maliformations of the external genitalia were found in 3 out of 7 male patients. Two of them presented with a bifid scrotum and a caudally displaced, dysplastic penis. Two patients had been exposed to progestin in early pregnancy and one of them had hyposadias and testicular atrophy. It is suggested that malformations of the male external genitalia might be part of malformations found in the VATER association.
