BACKGROUND AND PURPOSE: Maintaining long-term peritoneal catheter function for peritoneal dialysis is commonly threatened by problems with catheter obstruction. Multiple methods have been used to salvage nonfunctioning catheters, including omentopexy, catheter repositioning, and omentectomy. We report on our experience with a laparoscopic method of omentectomy and catheter fixation for salvage of nonfunctioning peritoneal dialysis catheters. PATIENTS AND METHODS: Thirteen patients with nonfunctioning peritoneal dialysis catheters underwent 16 laparoscopic procedures with the intent to restore function...

OBJECTIVE: To describe a technique and results of uterine suspension and positioning by extraperitoneal ligament investment, fixation and truncation (UPLIFT). STUDY DESIGN: Seventy-five women aged 19-48 years in a private referral center with chronic pelvic pain, dyspareunia and dysmenorrhea seeking treatment were evaluated and treated over a two-year period. Laparoscopic uterine suspension was performed using the Carter-Thomason 2-mm needle point suture passer...

Inguinal and incisional hernias are frequently discovered during gynecologic laparoscopy. Current laparoscopic techniques to repair hernias are time consuming and require expensive procedure-specific instrumentation. The Pelosi simplified method of hernia repair, which uses an operative laparoscope, Carter-Thomason needle carriers, and standard suturing material, was performed successfully in eight women to repair inguinal hernias at the time of gynecologic laparoscopy. No intraoperative or postoperative complications occurred...

Postoperative omental and bowel entrapment secondary to incisional hernias and life-threatening hemorrhage secondary to anterior abdominal wall bleeding may occur during cannula placement. The Carter-Thomason needle suture passer was used to correct 20 fascial and peritoneal cannula defects and 7 severe anterior abdominal wall bleeders. The procedure was performed quickly and successfully in all instances. The instrument provides a safe, rapid, easy, and cost-effective way to correct complications secondary to the creation of surgical ports during laparoscopic surgery...

An operative technique is described for fasciorrhaphy of large laparoscopic incisions, umbilical hernias or small abdominal wall hernias with laparoscopic monitoring within the scope of minimal invasive surgery. Over a period of 8 months the Carter-Thomason technique was used in 105 operative laparoscopies for occlusion of large laparoscopic incisions and in 6 pre-existing umbilical hernias. In the postoperative period no hernias occurred and no complications were observed.

RANDOMIZED CONTROLLED TRIAL

BACKGROUND: Recently, a number of laparoscopic port-closure techniques have been reported to avoid the complications associated with the port closure after laparoscopic surgery. To evaluate these port-closure techniques, we compared seven new laparoscopic port-closure techniques with the standard technique of a hand-sutured closure. STUDY DESIGN: In a prospective, randomized study, 95, 12-mm port sites in 32 patients undergoing transperitoneal laparoscopic procedures were randomized to one of eight different port-site closure techniques...

COMPARATIVE STUDY

Obtaining a precise fascial closure of larger laparoscopic port sites presents an ongoing challenge for even the most experienced laparoscopic surgeons. To this end, we evaluated five new methods for closing laparoscopic port sites and compared these with the standard technique of a hand-sutured repair using a 0 absorbable suture in the pig. The new devices studied were the Carter-Thomason Needle-Point Suture Passer (CTNSP), the eXit Disposable Puncture Closure (EDPC) device, the Maciol suture needle (MSN) set, the Endoclose suture carrier, and a nonpropreitary technique using a long 14-gauge Angiocatheter and a looped Prolene suture...

STUDY OBJECTIVE: To determine the frequency of driving while impaired (DWI) charges among alcohol-intoxicated drivers injured in motor vehicle crashes (MVCs) and any differences in the group of those charged compared with those not charged. METHODS: We performed a retrospective analysis of linked data from medical and judicial sources. Our setting was an urban emergency department of a trauma center serving a population of 1 million. We studied consecutive drivers injured in MVCs over a period of 15 months who had measured serum ethanol (BAC) levels of 100 mg/dL or higher...

The objective was to develop a technique and instrumentation for closure of 10-mm and larger laparoscopic incisions with a single instrument under direct laparoscopic vision, ensuring complete fascial and peritoneal closure. The Carter-Thomason Needle-Point Suture Passer functions as both a needle and a grasper, allowing for performance of laparoscopic directed fascial and peritoneal closure. It uses a 2.7-mm diameter grasping tool with a single-action jaw. The device introduces the suture through the muscle, fascia, and peritoneal layers under direct laparoscopic vision, drops the suture, picks it up at the opposite side of the opening, and is withdrawn grasping the suture...
