OBJECTIVES: Heteroresistant infections are defined as infections in which a mixture of drug-resistant and drug-susceptible populations are present. In Mycobacterium tuberculosis ( M. tb ), heteroresistance poses a challenge in diagnosis and has been linked with poor treatment outcomes. We compared the analytical sensitivity of molecular methods, such as GeneXpert and whole genome sequencing (WGS) in detecting heteroresistance when compared with the 'gold standard' phenotypic assay: the agar proportion method (APM)...

Mycobacterium avium complex pulmonary disease is treated with an azithromycin, ethambutol, and rifampicin regimen, with limited efficacy. The role of rifampicin is controversial due to inactivity, adverse effects, and drug interactions. Here, we evaluated the efficacy of clofazimine as a substitute for rifampicin in an intracellular hollow-fiber infection model. THP-1 cells, which are monocytes isolated from peripheral blood from an acute monocytic leukemia patient, were infected with M. avium ATCC 700898 and exposed to a regimen of azithromycin and ethambutol with either rifampicin or clofazimine...

In pediatric Acute Lymphoblastic Leukemia (ALL), mutations/deletions affecting the TP53 gene are rare at diagnosis. However, at relapse about 12% of the patients show TP53 aberrations, predicting a very poor outcome. Since p53-mediated apoptosis is an endpoint for many cytotoxic drugs, loss of p53 function frequently leads to therapy failure. In this study, we show that CRISPR/Cas9-induced loss of TP53 drives resistance to a large majority of drugs used to treat relapsed ALL, including novel agents such as inotuzumab ozogamicin...

OBJECTIVES: Cytotoxic T cells and natural killer (NK) cells are central effector cells in cancer and infections. Their effector response is regulated by activating and inhibitory receptors. The regulation of these cells in systemic autoimmune diseases such as systemic sclerosis (SSc) is less defined. In a pilot study of telephone follow-up as a part of the sepsis care process, no direct patient benefit could be demonstrated. A nurse with competency in infectious diseases called patients two to six weeks post-discharge after inpatient sepsis care to assess the need for further medical attention. Out of 51 eligible patients, only 21 telephone follow-ups could be realized. Supporting recommendations could be given in nine cases, but no direct medical advice was given. The follow-up was resource intensive; for each supporting recommendation, time used was calculated to 279 minutes...

The rise of CO2 concentrations in the environment due to anthropogenic activities results in global warming and threatens the future of humanity and biodiversity. To address excessive CO2 emissions and its effects on climate change, efforts towards CO2 capture and conversion into value adduct products such as methane, methanol, acetic acid, and carbonates have grown. Frustrated Lewis pairs (FLPs) can activate small molecules, including CO2 and convert it into value added products. This review covers recent progress and mechanistic insights into intra- and inter-molecular FLPs comprised of varying Lewis acids and bases (from groups 13, 14, 15 of the periodic table as well as transition metals) that activate CO2 in stoichiometric and catalytic fashion towards reduced products...

Seahorse is a valuable marine-animal drug widely used in traditional Chinese medicine (TCM), and which was first documented in the "Ben Cao Jing Ji Zhu" during the Liang Dynasty. Hippocampus kelloggi (HK) is the most common seahorse species in the medicinal material market and is one of the genuine sources of medicinal seahorse documented in the Chinese pharmacopeia. It is mainly cultivated in the Shandong, Fujian, and Guangxi Provinces in China. However, pseudo-HK, represented by Hippocampus ingens (HI) due to its similar appearance and traits, is often found in the market, compromising the safety and efficacy of clinical use...

Treatment of skin and soft tissue infections with nontuberculous mycobacteria sometimes fails despite repeated debridements and long-term systemic antibiotic therapy. These treatment-refractory infections can cause significant morbidity and pose a treatment challenge. Following surgery, we treated three patients with negative pressure wound therapy with the instillation and dwell time of topical antibiotics, in addition to systemic antibiotic treatment. Treatment was successful and well tolerated, except for some local irritation...

Bullous pemphigoid affects people who are usually very frail, because they are very old and frequently have neurological co-morbidities. Over the past twenty years, therapeutic management has improved thanks to the application of highly potent corticosteroid-based creams, following a well-established regimen, for several months. Well-conducted nursing care is therefore essential not only to cover wounds, but also to treat the disease. Dedicated nursing time is also an opportunity to provide support to patients, who are often very uncomfortable...
