UNLABELLED: This is an update of the 2015 Guidelines developed by the Japan Otological Society and Oto-Rhino-Laryngeal Society of Japan defining otitis media with effusion (OME) in children (younger than 12 years old) and describing the disease rate, diagnosis, and method of examination. Recommended therapies that received consensus from the guideline committee were updated in consideration of current therapies used in Japan and based on available evidence. METHOD: Regarding the treatment of OME in children, we developed Clinical Questions (CQs) and retrieved documents on each theme, including the definition, disease state, method of diagnosis, and medical treatment...

There is a considerable interest in understanding transient human upper airway aerodynamics, especially in view of assessing the effects of various ventilation therapies. Experimental analyses in a patient-specific manner pose challenges as the upper airway consists of a narrow confined region with complex anatomy. Pressure measurements are feasible, but, for example, PIV experiments require special measures to accommodate for the light refraction by the model. Computational fluid dynamics can bridge the gap between limited experimental data and detailed flow features...

Laryngeal cleft is a rare pathology and needs a high index of suspicion for diagnosis. Various classifications are used but the Inglis classification which describes 4 grades of cleft is most widely accepted. Grade 3 and 4 clefts are very rare and are usually associated with other congenital abnormalities. Grade 1 and 2 clefts are more common and can be easily corrected with good outcomes. We are presenting our experience with 9 cases of low grade (Grade 1 and 2) laryngeal cleft which were managed successfully in our department...

OBJECTIVES: To describe the spectrum of swallowing abnormalities in children with Type I laryngeal cleft (LC-1) and evaluate the effect of LC-1 repair on swallowing abnormalities. METHODS: A retrospective review was performed of all consecutive children who were diagnosed with LC-1. Swallowing function was evaluated pre- and post-operatively using video fluoroscopic swallow study (VFSS). VFSS reports were used to define swallowing abnormalities and to determine penetration aspiration scale (PAS) and functional oral intake scale (FOIS)...

OBJECTIVES: The traditional endoscopic techniques for surgical management of laryngeal clefts are carbon dioxide (CO2 ) laser or microlaryngeal instruments (cold steel). This study compares the functional efficacy and safety of coblation, or "cold" radiofrequency ablation, to traditional approaches for endoscopic laryngeal cleft repair. METHODS: Patients who underwent endoscopic laryngeal cleft repair with CO2 laser, cold steel, or coblator at two tertiary academic centers from 2015 to 2021 were retrospectively identified...

Chronic cough is defined as cough lasting more than 4 weeks in children aged 14 years or older. Normal children, without pathophysiology, can cough up to more than 30 times a day. When cough occurs pathologically, it is often more often and can be divided into specific and nonspecific cough types. Inputs from otolaryngology, pulmonary medicine, and gastroenterology, along with other specialties in an aerodigestive team setting, allow a team approach to consider a wide variety of causes of cough and coordinate diagnostic procedures with treatment...

Objective: Flexible bronchoscopy is widely used in infants and it plays a crucial role. The aim of this study was to investigate the value and clinical safety of flexible bronchoscopy in a neonatal intensive care unit. Methods: A retrospective analysis was performed on the clinical data of 116 neonates who underwent flexible bronchoscopy and the outcomes of 147 procedures. A correlation analysis was performed on the relationship between flexible bronchoscopy findings, microscopic indications, and clinical disease...

OBJECTIVE: Laryngoscopy corresponding with laryngeal electromyography (LEMG) is essential in diagnosing vocal fold paresis. However, baseline asymmetry or other diseases oftentimes confound the exam, making diagnosis difficult. There is currently no agreed upon endoscopic criteria proven to reliably correlate with LEMG findings. We define a set of endoscopic findings termed "paresis triad" that, when present together, reliably correlate with LEMG. The paresis triad consists of (1) hypocontraction of the weak side of the larynx with increased ventricular show, (2) hypercontraction of the intact side with bulging of the false fold covering the ventricle, and (3) tilting of the interarytenoid cleft to the weak side...

Introduction: Laryngeal clefts (LC) are upper respiratory malformations predominately found in the posterior laryngeal wall. The frequency is 1:10,000, more frequently affect males, and can be syndromic features. There is no report of a transfixing anterior laryngeal cleft. Case report : This diabetic mother at full-term pregnancy delivered a stillborn macrosomic 4780 g dysmorphic stillborn male with left renal agenesis, aortic coarctation, and anterior laryngeal cleft. Conclusion : Anterior laryngeal clefts can occur, and in this case, occurred in association with renal agenesis and maternal diabetes...

Our goal was to evaluate combined LCIL and Sal-Bot and create recommendations for further treatment based on response to initial treatment.
