Hiv primoinfection

J Rajcani
Immunosuppression (ISP) affects the outcome of acute virus primoinfection as well as the course of virus latency. ISP may result from viral infection of the immune cells. This infection can be either transient, as in measles virus infection, or progressive as in Human immunodeficiency virus (HIV-1, 2) infections, which proceed to acquired immunodeficiency syndrome (AIDS). Innate ISP occurs due to various congenital defects of cells, participating within immune response. In the majority of cases, ISP may be initiated by physical causes (e...

To analyze superinfection in an HIV-1-infected patient showing high-risk practices, viral quasispecies were analyzed in pol and env genes in several plasma samples. Phylogenetic analysis in the reverse transcriptase fragment in pol gene identified a single virus in the first 3 samples analyzed, but 12 years after primoinfection, 3 different viral strains were detected in the patient quasispecies. This result suggests a superinfection with 2 HIV-1 strains, one of which showed the T215Y + M184V resistance mutations...

We have performed a retrospective study of 61 cases of Pneumocystis carinii pneumonia (PCP) among the Human Immunodeficiency Virus (HIV) seropositive population at the 11th Health Area of the Comunidad de Madrid with microbiological diagnosis until January 1994. We made an Epidemiologic analysis of this PCP group and compared it with the Epidemiologic data of the general HIV population in Madrid. Besides, we studied the radiologic manifestations and their possible associations with several factors (sex, HIV risk factor, time of HIV risk practice, time of HIV infection, associated infections, number of CD4 lymphocytes, radiologic secuelae, time of healing, mortality, number of PCP episodes, time interval from primoinfection to death)...

BACKGROUND: Infection by the varicella-zoster virus (VZV) is frequent in patients infected by the HIV-1. Nonetheless, visceral involvement in addition to that of pneumonia is rare, despite the important immune dysfunction found among these patients. METHODS: Varicella pneumonia was diagnosed in 2 patients with HIV-1 infection who presented cough with high fever and a characteristic rash in addition to respiratory failure and a micronodular pattern on chest radiography...

One mechanism proposed to play a role in T-cell depletion in human immunodeficiency virus (HIV) infection is apoptosis (activation-induced cell death). We assessed whether apoptosis is related to activation of T cells in vivo and its possible triggers. DNA was extracted from peripheral blood mononuclear cells (PBMC) taken from 16 vertically HIV-infected children and 9 HIV-negative children born to HIV-positive mothers (controls) and tested by agarose gel electrophoresis for the presence of DNA fragments specific for apoptosis...

A woman who had recently become infected with HIV-1 after sexual relations with a seropositive subject presented a rapid fall in CD4+ T lymphocytes and evolved to AIDS 13 months after primoinfection. The most plausible cause for the rapid disease progression in this patient was the observation of a viral syncytium forming phenotype. The plasmatic viremia was high throughout the follow up despite the administration of antiretroviral drugs.

Using the marker Leu2 + Leu7 + to detect the CD8 suppressor cells (CD88+CD57+), we have observed that their blood level, which increases after primoinfection, during the latent period, increases still further just before and when the p24 antigen returns and is present in the blood. Its ratio over cytotoxic CD8+ cells remains high during the AIDS period, though the absolute number decreases.

We report a case of acute regressive meningoradiculitis concomitant with HIV1 primoinfection. The clinical symptoms were mild and disappeared spontaneously. Electromyographic studies confirmed the regressive demyelinating-type process. The biological diagnosis of HIV1 infection was demonstrated by viral antigen detection, by the presence of the integrated proviral DNA after gene amplification by a polymerase chain reaction, and in particular, by the steady progression in the appearance of different HIV1-specific antibodies...
