BACKGROUND: The usefulness of fine needle aspiration cytology (FNAC) in the diagnosis of lung lesions is well documented. Fungal lesions are among nonneoplastic lesions of the lung in which FNAC has proven a useful technique in both immunocompromised and immunocompetent patients. These include cryptococcosis, aspergillosis, histoplasmosis and coccidiodomycosis. Pulmonary mucormycosis, an aggressive fungal infection, is rarely diagnosed on FNAC. We report a case of isolated pulmonary mucormycosis diagnosed on FNAC...

The incidence of coccidiodomycosis in the southwestern United States has sharply increased over the last decade as a result of environmental and demographic changes. It is important to review the epidemiology, diagnostic indicators, and therapeutic options for both immunocompetent and immunocompromised patients who have a particularly high risk of infection.

CYTOMEGALOVIRUS INFECTION: Primary prophylaxis is logical in high-risk subjects (CD4 < 50) who are asymptomatic; per os ganciclovir is under evaluation. Systematic secondary prophylaxis relies on intravenous ganciclovir or foscarnet. HERPES SIMPLEX AND ZOSTER: Secondary prevention with acyclovir is not recommended due to the risk of resistance. Herpes zoster retinitis is an absolute indication for continuous oral treatment with acyclovir. CRYPTOCOCCUS: Oral fluconazol is required for secondary prophylaxis...

Coccidioides immitis, the causal agent of coccidiodomycosis, is endemic in the arid desert regions of Mexico and the southwestern United States. Individuals may acquire the disease through the inhalation of conidia. The majority of infected patients are asymptomatic or exhibit flu-like symptoms. Two percent of infected individuals ultimately demonstrate a solitary pulmonary nodule that may be radiographically indistinguishable from neoplasms or other infectious lesions. This report describes the spectrum of cytologic findings in 73 patients who were diagnosed with pulmonary coccidiodomycosis by fine-needle aspiration (FNA) biopsy...

Primary pulmonary coccidioidomycosis is usually a self-limited disease. It is characterized by fever, nonproductive cough and chest pain. Manifestations of hypersensitivity, such as skin lesions and polyserositis, are often present. Persistence of symptoms beyond eight weeks, continuously positive sputum cultures or negative skin tests with high serum complement-fixation titers for coccidiodomycosis suggest local progression or hematogenous spread. These situations are indications for antifungal therapy.

Fifteen cases of disseminated cerebral coccidioidomycosis (DCC) were evaluated by computed tomography (CT). Of the 14 patients with abnormal CT scans, all but 2 showed definite evidence of hydrocephalus. Abnormalities of the basal cisterns were present on non-contrast scans in 8 cases (53%). Ten (67%) demonstrated abnormal contrast enhancement of basal cisterns and other cerebrospinal fluid (CSF) spaces. White-matter lesions were detected in 6 cases (40%). Hydrocephalus usually follows abnormal enhancement of basal cisterns...

Adherence of various combinations of polymorphonuclear leukocytes (PMN) or alveolar macrophages (AM) and serum from nonimmunosuppressed patients wit culture-proved, invasive fungal infections or control subjects was evaluated in vitro using the standard nylon fiber pipette technique. In autologous serum, adherence of PMN from patients with wide variety of untreated fungal infections, including blastomycosis, histoplasmosis, cryptococcosis, coccidiodomycosis, and spirotrichosis, was significantly (p less than 0...

Among Piman and San Carlos Apache Indians, the rates of disseminated disease and death from coccidioidomycosis have declined 67 percent (p less than 0.001) and 71 percent (p less than 0.01), respectively, between the first and second half of a 22-year observation period (1959-1980), despite a lack of significant change in the rate of primary infection (as determined by the coccidioidin skin test) between the two 11-year periods. The two tribal groups studied comprised 76 percent of the Indian population in the endemic area...

A review of diseases of dogs of public health significance in Nigeria is given based essentially on recent findings. The objective is to collate and inform the public of some Zoonotic diseases which have been reported in owned and stray dog populations in Nigeria so as to encourage better methods of pet health care delivery and control in the country. Rabies, Q fever, Toxoplasmosis, Brucellosis, Salmonellosis, Coccidiodomycosis, Ring worm and Helmithzoonotic parasites associated with Ancylostomiasis, Echinococcosis, Toxocariasis and Cestodiasis are among diseases transmissible from dogs to man in Nigeria...
