Primary tracheal carcinoma is one of very rarely seen tumors and the rate of its being seen makes up approximately % 0.2 of all tumors of respiratory tract. A 50-year-old man, with a history of right lower lobectomy for pulmonary adenocystic carcinoma, presented with chronic flank pain due to a large mass in the right kidney. He also had bilateral pulmonary nodules and cardiac masses, indicating metastatic disease. Simultaneous metastatic involvement of the kidney and the heart from a rare pulmonary tumor made this case unique in the prevailing medical data.

The present study was addressed to find out the expression of Bcl-2 proto-oncogene in tumor tissue derived from 24 patients with malignant oral carcinoma and oral mucosa from the same patients served as control and showed a cytoplasmic pattern of Bcl-2 immunoreactivity in basal cell layer. Fourteen of 24 (58.3%) of oral carcinoma and four adenocystic carcinoma expressed positive Bcl-2 oncogene. Well-differentiated squamous cell carcinoma showed absence of immunoreactivity. Forty-two cases of orbital lesions in superolateral quadrant were evaluated in our study by fine needle aspiration biopsy. Cases were histologically proven wherever possible. In 92.85% cases the diseases were correctly diagnosed. Females were mainly affected. Lymphomatous tumours were diagnosed on the basis of atypical lymphoid proliferation, correlated with clinical features and findings of computed tomographic scan and managed by radiotherapy. Reported is the successful treatment of a 24-year-old male with adenocystic carcinoma involving the tracheal carina, in which the tumor extended along the right main bronchus across the orifice of the right upper lobe. The patient underwent a carinal resection plus right upper lobectomy and reconstruction of the carina, resulting in neither anastomotic complication nor recurrence of disease during 28 months of follow-up.
