THE AIM: The aim of this thesis was to elucidate more differences between a familial and sporadic inflammatory bowel disease by comparing certain clinical data. METHODS: We assessed 248 patients with inflammatory bowel disease (IBD) observed in 1994-2004 in the Academic Department of Gastroenterology at the Medical Faculty in Hradec Králové. Crohn disease treatment is a considerable challenge for a medical doctor. Gaining clinical remission is not enough. The important factor taken into account in treatment is gaining clinical remission and complete mucosal healing. Treatment useful in mucosal healing consists of immunosupresants as azathioprine, 6-markaptopurin, methotrexate and biological treatment. Important factors influencing mucosal healing is choice of medical treatment and its proper timing. A new era of immunotherapy is arriving in medical therapeutics. Both immunosupresion and immunomodulation have advantages which the clinician must learn. In this review, the author describe several immunomodulators which include cytokines, interleukins, interferons, tumor necrosis factor, granulocyte colony-stimulating factor, erythropoietin, and immunoglobulins. Some of these therapeutics approaches are already with us, and some are at present in the experimental stage.
