OBJECTIVES: Presentation of a case of carotid stenosis in an elderly patient with multiple co-morbidities, and its management. METHODS: Pre-operative management of an elderly patient affected by carotid artery stenosis with cardiac, metabolic and renal comorbidities and a recent history of buccal squamous carcinoma. RESULTS: Pre-operative anestesiologic assessment showed an increased risk for the presence of heart failure, cardiogenic pulmonary hypertension, mild/moderate stage renal disease...

BACKGROUND AND OBJECTIVE: Optimization of the preoperative hemoglobin (Hb) level is an effective way to reduce allogeneic transfusion in total knee arthroplasty (TKA) though the procedure is expensive, requires close monitoring and is often inconvenient for patients with reduced mobility. Our aim was to investigate the value of preoperative Hb levels to predict transfusion and thereby tailoring Hb optimization to patient characteristics. MATERIALS AND METHODS: All consecutive patients who undergone primary TKA in our center over 2 years, and received tranexamic acid intraoperatively, were reviewed...

REVIEW
G Lucarelli, A Breda
Since the first successful stone extraction through a nephrostomy in 1976, percutaneous nephrolithotomy (PCNL) has became the preferred procedure especially for treatment of large, complex and staghorn calculi. For decades this method has been performed with the patient in the prone position. More recently, particular interest has been taken on supine PCNL due to less anestesiological risks and the possibility of simultaneous anterograde and retrograde access to the whole urinary tract. Although many retrospective studies have been published, only two prospective trials comparing the two positions are reported in the literature...

Endogenous Cushing's syndrome is a grave disease that requires a multidisciplinary and individualized treatment approach for each patient. Approximately 80% of all patients harbour a corticotroph pituitary adenoma (Cushing's disease) with excessive secretion of adrenocorticotropin-hormone (ACTH) and, consecutively, cortisol. The goals of treatment include normalization of hormone excess, long-term disease control and the reversal of comorbidities caused by the underlying pathology. The treatment of choice is neurosurgical tumour removal of the pituitary adenoma...

INTRODUCTION: Cost of anaesthesiology represent defined measures to determine a precise profile of expenditure estimation of surgical treatment, which is important regarding planning of healthcare activities, prices and budget. OBJECTIVE: In order to determine the actual value of anaestesiological services, we started with the analysis of activity based costing (ABC) analysis. METHODS: Retrospectively, in 2005 and 2006, we estimated the direct costs of anestesiological services (salaries, drugs, supplying materials and other: analyses and equipment...

Studies till now conducted about operating rooms' microclimate have been focused mainly on operators' thermal comfort, considering that uneasiness conditions may compromise their working performance. In last years, nevertheless, the anesthesiologic community recalled attention on patients' risks determined by perioperatory variations of normothermia, underlining the necessity of orientating studies to individuate microclimate characteristics act to guarantee thermal comfort of the patient too. Looking at these considerations, a study has been conducted in the operating rooms of the hospital-university Firm and the n...

UNLABELLED: Obstructive sleep apnea (OSA) caused by enlarged tonsils and adenoids is common in pediatric population. The prevalence of pediatric obstructive sleep apnea syndrome has been estimated to be between 1% and 3% in preschool and school-aged children. The aim of this study was to examine quality of life in children before and after adenotonsillectomy. SUBJECTS AND METHODS: This study was prospective and it was carried out in the period from mid-November 2005 to end-June 2006...

Laparoscopic surgery is a technique by which abdominal surgical treatments are performed in minimally invasive way. Many operations that once required long hospitalization are now being performed with laparoscopic technique. It has significantly reduced postoperative pain and the number of complications, shorter hospital stay, with faster recovery, and more rapid return to normal activities. These are only some of the advantages of laparoscopic surgery. The first laparoscopic cholecystectomy in Croatia was performed on May 14, 1992, at the University Department of Surgery of "Sveti Duh" General Hospital in Zagreb...

"The time will come when German medicine, too, will have to concern itself with the issue of a professional narcotiseur. Until then it will be our duty to keep the interest in narcosis, which has increased satisfactorily in the past years, alive." With this statement the editors of the journal "Der Schmerz" substantiated the publication og a German-speaking anestesiological journal in 1928. Ernst von der Porten, a professional anesthesist working in Hamburg was the chief initiator for the appearance of the new journal...

The action of 2-3 mg/kg of morphine on the threshold of electrical fibrillation of the ventricle (TVF) in anesthetized dogs has been studied. The basal TVF was 10.5 +/- 4.9 mA, increased to 20.2 +/- 7.7 mA (P 0.005) with the administration of this drug. Comparatively, the effect of morphine on the TVF is similar to that of propranolol and bretilium and more effective than procainamide. The mechanism by which it acts is not well known. Perhaps it is due to a homogenization of refractory periods in the ventricle as well as a possible morphine-catacholamine interaction...
