Verneuil's disease or perianal hidradenitis suppurativa is a chronic suppurative disease with a tendency to fistulization and sclerosis, developing on the subcutaneous tissue, on apocrine glands. The perianal location is the most frequent, and the elective therapy in such case is surgery. A case of perianal Verneuil's disease of great size, invalidating for the patient, in which medical therapy failed, making necessary an aggressive surgical treatment is reported. PURPOSE: The authors describe the clinical and bone scintigraphic findings of the SAPHO syndrome, which is characterized by synovitis, acne, palmoplantar pustulosis, hyperostosis, and osteitis. The case report illustrates the significance of bone scintigraphy in the diagnosis. It shows that Tc-99m MDP scanning can detect signs of arthritis not seen with other imaging methods, because the arthritis is inflammatory in nature and does not always cause bone erosion. A 20-year review of the inflammatory bowel disease surgical database of the author was analyzed for Crohn's disease (CD) patients who had a surgical approach to perianal fistula disease (PAD). Of 333 patients with CD operated between July 1977 and February 1997, 51 had procedures for PAD (15.3%), and 7 of these patients had laser ablation of severe, debilitating complex PAD (13.7%). These patients have traditionally been treated by diverting ileostomy or proctectomy with permanent diversion. Others have advocated conservative management with long-term antibiotics, staged operations, and insertion of multiple loose setons to promote drainage...

The antiinflammatory roentgenotherapy was used in 150 patients with pyo-inflammatory processes (post-operative inflammatory complications, periappendicular infiltration, hydradenitis, osteo-articular panaritium, mastitis). Good and satisfactory results were obtained in 92,6% of the patients. Positive dynamics of the T-lymphocyte level and increased activity of the blood serum lysozyme proved to be good prognostic signs.

In a 59 year-old woman, polychemotherapy with cyclophosphamide and mitoxantrone was started because of liver and bone marrow metastases following operative treatment of breast cancer with subsequent X-ray treatment in 1982. Three weeks after the fourth cycle of polychemotherapy multiple small, firm, reddish papules and pustules developed on the lymphedematous right arm. Histology revealed a dense, well-circumscribed granulocytic infiltrate around the acrosyringial part of the sweat gland duct. A total of 2820 oilmen regularly exposed to industrial oil were examined. Folliculites were detected in 151, furuncles in 63, hydradenitis in 58, phlegmons in 35, panaritium in 30 subjects. The number of invalidity days due to pyogenic dermatoses was 2-3 times higher among the workers exposed to deep oils than among those handling surface oils; this difference is explained by chemical composition and physicochemical characteristics of the extracted oil.

A case of squamous cell carcinoma arising in hydradenitis suppurativa of over 20 years duration is described. Only 6 other cases are reported in the literature. Multiple sites of origin of malignancy is suspected in this case. Wide-spread bone metastases resulted in severe hypercalcemia. Palliation was obtained for a short period following chemotherapy and Adriamycin and Bleomycin. Autopsy studies showed metastatic tumor in almost all the organs of the body.
