OBJECTIVE: To investigate the effect and mechanism of interleukin-8 expression induced by wood smoke particles (WSP) in primary human airway epithelial cells. METHODS: Primary human bronchial epithelial cells (HBEC) were collected through fiberbronchoscopic brushing and incubated with different concentration of WSP (0, 12.5, 25.0, 50.0, 100.0 μg/ml). Levels of IL-8 protein in cell culture supernatant were measured using enzyme-linked immune sorbent assay. The specific inhibitors for epidermal growth factor receptor (EGFR) and p38 kinase signaling pathways were employed to pretreat HBEC, respectively, prior to incubation with 100 μg/ml WSP to investigate the mechanism of IL-8 expression induced by WSP...

BACKGROUND: We set out to observe the morphologic changes and determine prognosis based on airway epithelial injury after inhalation injury of varying severity using a fiberbronchoscope and to explore the relationship between the severity of epithelial injury and its clinical manifestation. METHODS: Sixty burn patients with an inhalation injury underwent fiberoptic bronchoscopy. Morphologic changes of the airway epithelium were observed, and the inhalation injury was divided into 3 categories based on the severity of the epithelial injury: first-degree injury, in which the airway epithelium had slight hyperemia and edema; second-degree injury, in which obvious epithelial hyperemia and edema, epithelial erosion, or petechial hemorrhage was observes; and third-degree, in which the airway epithelium had necrosis and exfoliation...

BACKGROUND AND OBJECTIVES: Moebius sequence (MS) is a rare paralysis of the VI and VII cranial nerves. Craniofacial changes, which can hinder tracheal intubation considerably, are seen in approximately 90% of the patients. CASE REPORT: A male patient, 2 years and 5 months old, with MS, underwent flexible bronchoscopy for evaluation of laryngotracheomalacia. Comorbidities: bronchospasm and interventricular communication. Anesthesia was induced with sevoflurane in 100% O2, followed by venoclysis...

BACKGROUND: Usually, transbronchial biopsy (TBB) is performed in the evaluation of bronchoscopically invisible solitary nodules and peripheral lesions of the lung. Additionally catheter aspiration can be done with the fiberbronchoscope, also under fluoroscopic guidance, to get cytological material. Despite of its simplicity, the catheter aspiration technique is widely unknown. METHOD: In a prospective study we compared the diagnostic yield of both procedures concerning malignancy...

In non-smokers the underlying causes for chronic persistent cough (CPC) e.g. chronic cough without diagnostic chest X-ray or pulmonary function test--are usually as follows: several common upper airways diseases, bronchial (cough type) asthma, gastrooesophageal reflux or treatment with an ACE (angiotensin converting enzyme)--inhibitor. In 10% of CPC however the cause remains uncertain. We report a 30 year old non-smoker with severe coughing and repeated vomiting for two months. No laboratory or technical data could be collected suggestive of a common cause of CPC: Upper airways disease, bronchial flow limitation or hyperresponsiveness, ACE inhibitor medication, B...

General anesthesia was scheduled in a patient who had undergone radical surgical resection because of his lower lip squamous cell carcinoma. His oral area had been reconstructed drastically and this pathological change indicated difficulties of mask fitting, mask ventilation and laryngoscopic intubation. Before induction of general anesthesia, we performed fiberbronchoscopic transnasal awake intubation. The patient's condition was stable during both intubation and operation. At the end of anesthesia, the tracheal tube was removed after the patient had become fully awake and had recovered completely from neuromuscular brockade...

Gaucher disease is the most common of the glycolipid storage disorders caused by the deficiency of glucocerebrosidase, an enzyme which hydrolyzes glucocerebroside to glucose and ceramide. Accumulation of the substrate leads to multiorgan dysfunction involving the brain, spleen, liver, lymph node and bone marrow. In anesthetic management, gastroesophageal reflux leading to recurrent aspiration, pancytopenia and airway difficulties due to trismus, neck extension and upper airway infiltration with glucocerebroside must be considered...

Silicone and metal stents are available for the treatment of malignant bronchial stenoses. This project sought to compare the self-expanding nitinol Accuflex stent (Boston Scientific Corp; Watertown, Mass) with the passively expandable tantalum Strecker stent (Boston Scientific Corp; Watertown, Mass), both implanted by flexible bronchoscopy under local anesthesia and sedation. In 51 patients with malignant bronchial stenosis, 14 nitinol and 51 tantalum stents were used and stenoses of 75 to 100% were treated...

We compared the clinical usefulness of two double-lumen endobronchial tubes, one furnished by Concord Portex and the other Broncho-Cath by Mallinckrodt. Only left-sided 37 french size tubes were used. The difficulty of intubation, the opening state of the bilateral lumen in a bronchus, the displacement of the tube in changing a position, and easiness of passing the fiberbronchoscope or suction tube were compared in using the two double lumen endobronchial tubes. All parameters were slightly better in the Portex tube than in the Broncho-Cath, but the differences were not significant...

Small pulmonary lesions localized in peripheral airways or lung parenchyma are mostly not visible by means of flexible or rigid bronchoscopy for they are distal to the inspectable airway caliber. Therefore fluoroscopy is necessary to direct the flexible biopsy forceps or biopsy needle to the lesion. Even a two-dimensional fluoroscopic guidance does not guarantee an access to the focus. Therefore we investigated a method to overcome these problems. In 9 patients with peripheral lung lesions where the conventional method had failed to provide sufficient biopsies CT-guided bronchoscopy was done...

Thoracoscopy, using a fiberbronchoscope under open pneumothorax, was performed in 27 cases suffering from pleural diseases undiagnosed. The overall diagnostic rate was 92.3%. The items observed included pulse rate, respiration rate, blood pressure, blood gas analysis, electrocardiography and thoracic pressure measure. It was found that this method has little influence on cardiopneumatic function, no harm fiberbronchoscope, and is simple and safe.
