Herein, we present a neonatal case of coarctation of the aorta, with aortic arch thrombus confirmed by echocardiography. We performed thrombus removal and aortic arch repair emergently. This critical condition necessitates quick preoperative evaluation with echocardiography. Moreover, postoperative evaluation using computed tomography is reasonable to assess an aortic arch configuration, and exclude the remnant thrombus.

Spontaneous thrombosis in the aortic arch is a rare finding in the neonate. Often the thrombosis is initially interpreted as arch obstruction secondary to coarctation or interrupted aortic arch. Thus the obstruction is mechanical with no structural abnormality. We describe 2 newborns with coarctation and extensive thrombosis within the aortic arch. Umbilical artery catheterisation is considered the standard of care for arterial access in neonatal intensive care unit. It is routinely used for blood sampling and blood pressure monitoring. A 6-day-old neonate presented with hypernatremic dehydration and weight loss. Fluids were started for rehydration. He was subsequently found to have signs of reduced lower limb perfusion and investigations revealed an abdominal aortic thrombus and bilateral occlusion of renal arteries. Systemic thrombolysis led to complete resolution of renal artery thrombi with no major complications. A review of similar cases with hypernatremic dehydration and thrombosis is provided.

To our knowledge, this is the first report describing coronary artery occlusion in a pediatric patient with critical aortic stenosis. We hypothesize that thrombus formation was due either to in situ fresh thrombus formation resulting from low cardiac output or secondary to an atrial arrhythmia in the presence of severe left ventricular diastolic dysfunction that may have occurred during the flight to our facility.
