We reviewed 183 threaded rings at a mean 7 years follow-up. Poor résults are frequent: 34 % had been revised and 50 % have radiographic migration, essentially with proximal migration of the cup. Generally, we reoperated with other cememtless implants. The type of threaded cup does not matter (spheric or conic). Only the duration of follow up seems significant. We think that biomecanical strains, rather than physiology, explain these numerous migrations. We have therefore abandoned threaded cups in favour of porous-coated impacted cups...

INTRODUCTION: The authors review and analyze a series of patients treated by total hip arthroplasty with a Karl Zweymüller threaded cup. METHODS: In a group of 115 consecutive primary total hip arthroplasties with the Karl Zweymüller cementless prosthesis, 100 acetabular screw rings have been retrospectively studied with a 5.8 years average follow-up (range: 40 to 96 months). The authors report a new technique in arthrodesis of the sub-occipital spine which has been experienced since 1973 in case of metastases at CI-C2 level. An occipitovertebral metal plate, inverted Y shaped has been realised after anatomical and biomecanical studies.
