AIM: In the present study the effect of defibrotide, an antithrombotic and profibrinolytic agent, was investigated in patients with chronic venous insufficiency (CVI) due to deep vein obstruction and/or reflux (chronic deep vein insufficiency, CDVI). METHODS: The study was a multicenter, randomized, double blind placebo controlled trial in which only patients with CDVI confirmed by ultrasound were enrolled. All patients were treated with adequate elastic compression and randomized to receive either oral defibrotide (800 mg/die) or matching placebo for 1 year...

Mild to severe postthrombotic sequelae, including chronic pain, edema, and ulceration, arise in one third of patients short after deep vein thrombosis (DVT). Recurrent DVT is closely associated with the development of postthrombotic syndrome (PTS), whereas if the extent and location of DVT might be relevant remains unclear. Chronic venous hypertension and abnormal microvessel or lymphatic function also correlates with PTS. The diagnosis of PTS is based on clinical grounds only if patients report a history of documented DVT; otherwise, objective testing is required...

BACKGROUND: Post-thrombotic syndrome (PTS) is a long-term complication of deep vein thrombosis (DVT) characterised by chronic pain, swelling and skin changes in the affected limb. One in every three patients with DVT will develop post-thrombotic complications within five years. OBJECTIVES: To determine the relative effectiveness of, and the rate of complications using non-pharmaceutical interventions in patients with DVT in the prevention of PTS. SEARCH STRATEGY: The reviewers searched the Cochrane Peripheral Vascular Diseases Group Specialised Trials Register (last searched January 2003), and the Cochrane Central Register of Controlled Trials (CENTRAL) (last searched Issue 4, 2002)...

The aim of this study was to evaluate deep venous thrombosis (DVT) prophylaxis with specific elastic stockings in long-haul flights (11-13 hours), in high-risk subjects. A group of 300 subjects was included; 76 were excluded for several problems including concomitant treatments; 224 were randomized into two groups (stockings vs. controls) to evaluate prophylaxis with below-knee stockings. An exercise program was used in both groups. Scholl (UK) Flight Socks (14-17 mmHg of pressure at the ankle) were used. DVT was diagnosed with ultrasound scanning...

THE THREE PROBABLE FACTORS INVOLVED IN DEEP VENOUS THROMBOSIS ARE: (1) stasis, (2) alteration in the endothelium of the veins, and (3) alteration in the clotting mechanism of the blood.Control of stasis is assisted by elevation of the foot of the bed and by the use of washable elastic stockings and early ambulation, together with foot exercises. To prevent damage to the endothelium of the veins during operation, the heels should be elevated off the operating table by means of a small rolled towel in order to take weight off the calf muscles...

Superficial vein thrombosis (SVT) risk factors are close to those of venous thromboembolism (VTE). Diagnosis is made in a clinical setting but ultrasonography is useful to eliminate concomitant deep vein thrombosis (DVT). For SVT of the lower limbs, which is the main location, varicose veins represent the principal cause but underlying conditions (e.g.: autoimmune diseases, malignancy or thrombophilia) must be sought in idiopathic, migrant or recurrent SVT and in the absence of varicose veins. Concomitant DVT and pulmonary embolism can occur in approximately 15% and 5% respectively...

BACKGROUND: Incidence of perioperative pulmonary embolism (PE) has increased in Japan probably due to the modernized life style. As the mortality rate of PE is very high, its prophylaxis is important. Therefore, prophylactic strategies for PE were instituted in the entire hospital. METHODS: Elastic stockings (ES) for preventing perioperative deep vein thrombosis in lower extremities were worn from the morning of the operation until the beginning of ambulation. Intermittent pneumatic compression (IPC) apparatuses were used in combination with ES...

UNLABELLED: The LONFLIT1/2 studies have established that in high-risk subjects after long ( > 10 hours) flights the incidence of deep venous thrombosis (DVT) may be between 4% and 6%. The LONFLIT4 study was aimed at evaluating the control of edema and DVT prevention in low-medium-risk subjects. In this study prophylaxis of edema with specific travel stockings was evaluated in 2 separate studies involving flights lasting 7 hours and 10-12 hours. Part I. Subjects at low-medium risk for DVT were contacted; 55 subjects were excluded for several nonmedical, travel-related problems or inconvenient evaluation time; the remaining 211 were randomized into 2 groups to evaluate prophylaxis with elastic stockings in 7-8-hour, long-haul flights...

One of the most common postoperative complications is venous thromboembolism, a term encompassing deep vein thrombosis and pulmonary embolism. This article reviews the epidemiology, natural history, difficulties in diagnosis, and strategies for the prevention of postoperative venous thromboembolism. We thoroughly review the currently available methods for thromboprophylaxis including: early ambulation, elastic compression stockings, pneumatic compression devices, inferior vena cava filters, and a variety of pharmacologic agents such as unfractionated heparin, warfarin, aspirin, low molecular weight heparin, and pentasaccharides...

INTRODUCTION: The use of medications to prevent thromboembolic risk is recommended in spinal cord injury patients during the early phase, but there is no consensus on treatment duration and modalities. Differences in practice include choice of medication (heparin, low molecular weight heparin, inhibitors of platelet aggregation), treatment duration (three to six months or more), and criteria for discontinuation of treatment. GOALS: To determine usual practice modalities for prophylaxis of deep vein thrombosis in spinal cord injury patients, and to identify determinant criteria...

Anecdotal reports have suggested a link between flying and the development of blood clots. A recent prospective study looking at passengers before and after flying has demonstrated a much higher incidence of asymptomatic venous thrombosis than we expected. In the same paper, graduated elastic compression stockings reduced this incidence of asymptomatic venous thrombosis. Further studies are required to see how many passengers develop symptomatic deep vein thrombosis and to assess the overall incidence of venous thromboembolic disease...

Intermittent pneumatic compression, compressive elastic stockings and standard or low-molecular heparin administration are most effective. Prophylactic measures do not exclude completely possibility of venous thromboembolic complications, but permit to control reliably this danger...

One of every three patients with deep-vein thrombosis of the lower extremities will develop, within 5 years, post-thrombotic sequelae that vary from minor signs to severe manifestations such as chronic pain, intractable edema, and leg ulceration. The post-thrombotic syndrome (PTS) develops as a result of the combination of venous hypertension due to persistent outflow obstruction or valvular incompetence and abnormal microvasculature or lymphatic function. Among factors potentially related to the development of PTS, recurrent ipsilateral thrombosis plays a major role...
