BACKGROUND: Aim of this study was to assess the potential differences in respecting the principles of goal-directed perfusion between aortic valve replacement with sutureless vs conventional stented bioprostheses. METHODS: Data from 94 consecutive patients undergoing aortic valve replacement with Perceval sutureless valve, with or without concomitant CABG, were compared to a contemporary cohort of 116 patients receiving conventional stented bioprostheses. Propensity score matching was used to reduce selection bias...

OBJECTIVES: The development of new percutaneous and surgical techniques has reduced the risk associated with aortic valve replacement procedures. We present the results of a Spanish register after initiating a programme for sutureless prostheses in moderate-high-risk patients. METHODS: This prospective multicentre study was carried out from November 2013 to November 2016. Data were obtained from 448 patients in whom a Perceval S prosthesis was implanted. RESULTS: The mean age was 79...

BACKGROUND: Surgical treatment of active prosthetic aortic valve endocarditis presents a challenge for cardiac surgeons because of tissue friability and destruction caused by infection. Sutureless prostheses, such as the Perceval S® (LivaNova, Saluggia, Italy), have emerged as an option among the different surgical approaches for these complicated cases. METHODS: This study presents data from nine patients who underwent aortic valve re-replacement with the Perceval S® due to active prosthetic aortic valve endocarditis, between January 2014 and August 2016...

PURPOSE: To describe surgical outcomes and structural characteristics of intraocular lenses (IOLs) implanted with transconjunctival sutureless intrascleral (SIS) fixation in human eyes. DESIGN: Retrospective interventional surgical case series involving live and cadaveric human eyes. METHODS: In this study, we investigated the surgical outcomes and structural anatomy of secondary IOLs implanted with the SIS technique in human eyes. All cases involving SIS IOL fixation performed at a single academic center from January 1, 2012, through July 30, 2016, were reviewed to describe the surgical technique, common indications, clinical outcomes, and the rate of common operative complications...

We describe a technique of small-incision lenticule extraction (SMILE)-assisted sutureless corneal resurfacing with interface tattooing for superficial limbal dermoids. After excision at a plane minimally below surrounding normal cornea and sclera, a corneal tattoo powder is carefully applied within an inked circular outline of the intended corneal margin. Circularity and cosmesis are assessed in natural light with the microscope light off. Fibrin glue-assisted corneal resurfacing is performed with the lenticule...

We report a case of infective endocarditis on a Perceval S aortic valve bioprosthesis, and we describe a feasible and useful technique that can help remove the infected valve. These maneuvers consist of an "x-movement" reached with the aid of anatomical forceps. The risk of infective endocarditis in patients with prosthetic valves is a well-known phenomena in cardiac surgery, and it can represent a life-threatening event in many cases. According to guidelines, an emergency surgical intervention is required in case of high risk of embolization, vegetation superior than 1 cm or acute cardiac failure secondary to the acute valve dysfunction...

Total anomalous pulmonary venous drainage (TAPVD) is an uncommon cardiac defect in children. The mixed subset accounts for 5%-10% of the TAPVD and is variable in its anatomy. The outcomes associated with this subset of patients are rarely reported. A retrospective review of all patients with mixed TAPVD undergoing repair at a single institution (1984-2014) was conducted. A descriptive analysis was performed. Twenty-four patients underwent repair for mixed TAPVD (6 univentricular physiology, 18 biventricular physiology)...

Aortic valve replacement with biological heart valves remains the gold standard for treating operable older patients with symptomatic aortic stenosis. In the last years trans aortic valve implantation (TAVI) has been shown to be superior to medical treatment in high risks patients with severe aortic stenosis. The transfemoral TAVI route has been also found in randomized controlled trials to be as good if not superior to standard AVR at 5 years in high risk patients. In intermediate risks patients TAVI was found in randomized controlled trials as a noninferior alternative to surgery, with a different pattern of adverse events associated with each procedure...

BACKGROUND: Stentless aortic valves have been developed to overcome obstructive limitations associated with stented bioprostheses. A new concept of surgical sutureless (SU) bioprosthesis has been developed in the last decade. In this multi-institutional study we sought to compare hemodynamics of different bioprosthesis concept as transcatheter (TAVR), Livanova Perceval S sutureless valve and Freedom Solo Stentless (FS) valve in patients undergoing aortic valve replacement (sAVR). METHODS: From 2010 to 2016, 478 patients underwent sAVR with FS, 518 Perceval S and 367 patients underwent TAVR...

A new controversy is raising in cardiovascular surgery, whether or not the new generation transcatheter valves used for valve implantation are superior to surgical techniques used for traditional valve replacement. If such a debate is far from being an issue for mitral valve treatment, where minimal invasive, (robotic or video-assisted) mitral valve repair has become the gold standard for mitral valve treatment while transcatheter mitral valve solutions are lagging behind(1-4), there are many voices stating that the future of aortic valve treatment will mainly be transcatheter(5)...

INTRODUCTION: The aim of this study was to evaluate the effectiveness and safety of a new three-component 'bundle' for insertion and management of centrally inserted central catheters (CICCs), designed to minimize catheter-related bloodstream infections (CRBSIs) in critically ill children. METHODS: Our 'bundle' has three components: insertion, management, and education. Insertion and management recommendations include: skin antisepsis with 2% chlorhexidine; maximal barrier precautions; ultrasound-guided venipuncture; tunneling of the catheter when a long indwelling time is expected; glue on the exit site; sutureless securement; use of transparent dressing; chlorhexidine sponge dressing on the 7th day; neutral displacement needle-free connectors...
