The most challenging step in sutureless scleral fixation of intraocular lens (SFIOL) is exteriorization of haptics. The conventional handshake technique has a learning curve since it involves intraocular handing over of haptics from one forceps to another. We describe a surgical technique of pars plana posterior capsulectomy using a transconjunctival sutureless 2-port 27-gauge microincision vitrectomy system to deal with dense posterior capsule opacification (PCO). We present a case of transcatheter valve-in-valve replacement performed because of structural valve deterioration of a subcoronary implanted stentless aortic bioprosthesis.
