Specific characteristics of a sidebend cranial strain result from displacement of the skull base, i.e. the sphenoid and occiput. Sidebend strain has skeletal, facial and dental consequences which are described in this patient example. An asymmetric occlusion is commonly seen in conjunction with sidebend strain, where the posterior occlusion is more Class II on the sidebend side and more Class I on the opposite side. By addressing the cranial base problem with A.L.F. appliances, this case example demonstrates an effective means of restoring facial, structural balance, and correcting the malocclusion that accompanies the sidebend strain...

Current techniques for palatal expansion are reviewed. Pre-treatment asymmetry of the palate and maxillary arch is shown to be almost universal and is not randomly distributed. The use of a symmetrical expansion appliance does not necessarily result in a symmetrical arch. ALF appliances provide a means of achieving orthopedic, symmetrical expansion of the palate by using very light force. This is demonstrated in seven subjects. It is argued that rapid palatal expansion is an inappropriate, potentially iatrogenic procedure which no longer has a place in the orthodontic armamentarium...

The cranial strain known as torsion does not fit within the Angle Classification. This strain is described and how to identify it. Torsion may occur as the only strain or is more commonly superimposed on another strain. Recognizing the presence of a torsion has a significant bearing on treatment sequencing. An approach to treatment is demonstrated.
