The article presents the operation results of 380 patients (170 (44,7%) men and 210 (55,3%) women) with obstruction of pyeloureteral segment at the period from 1996 to 2014. The middle age was 43,2±7,6 years old. Primary strictures took place in 302 (79,5%) patients and recurrent strictures - in 78 (20,5%). Open and laparoscopic plastic operations were performed in 248 (65,2%), endoscopic surgery - in 112 (29,5%), organo-removal surgery - in 20 (5,3%) cases out of 380 patients with obstruction of pyeloureteral segment...

The article presents the results of laparoscopic pyeloplasty in 72 patients with primary stricture of pyeloureteral segment (PUS). The study included 34 men, 38 women; mean age was 32 +/- 4,6 years. The operation was performed by transperitoneal approach in side position of patients. 4 trocars were used. There were no conversions to open surgery. Urinary leak after surgery was observed in 4 patients; in 3 patients it has discontinued independently, and in one patient it has required a repeat laparoscopy and suturing of anastomotic defect...

We studied effectiveness of the "conflict" vessel transposition in operations for hydronephrosis due to a vasoureteral conflict in children. For 1999-2007 we operated 323 children with unilateral hydronephrosis in obstruction of the pyeloureteral segment (PUS). The "conflict" vessels (CV) as a cause of the obstruction were detected in 67 patients who were divided into two groups. Ureteral endoprosthesis results are presented for 22 patients (9 males and 13 females) with long strictures of the upper urinary tract (UUT). Stenosis was caused by rectal tumor in 2 patients, by cancer of the uterine cervix in 5 patients. In a female with Burneville-Pringle disease metallic prosthesis was set in the pyeloureteral segment (PUS) of the solitary right kidney. Six patients had long strictures of the abdominal part of the ureter, 4 had PUS strictures, 3--of the ureter of the transplanted kidney and 1 had stricture of ureterocalicoanastomosis after Neivert operation...

Endometriosis is a biologically benign albeit aggressive pathology marked by high local recurrences. Ureteral involvement accounts for only a minority of cases (0.1-0.4%) with often non-specific symptoms at clinical presentation and difficult preoperative diagnosis. Examination of hydrodynamic condition of the upper urinary tracts with step-by-step pyelomanometry and profilometry of the pyeloureteral segment in 26 children at the age of 6 to 14 years with hydronephrosis provided topical diagnosis of the obstruction level. The priority role of the pelvis in development of obstructive urodynamic disorders was ascertained in 9 children. In the rest 17 children urodynamic disorders were determined by a different degree of obstruction in the prepelvic ureter. The obstruction of the pielo-ureteral junction, in most of the cases, recognizes an intrinsic etiopathogenesis, the presence of a segment ureteral aperistaltic and alteration of the ureteral musculature. In 4 patients with cutaneous urinary diversion who underwent percutaneous ureteral stone removal, similar ureteral complications developed as a result of severe ureteritis at the site of the stone. Ureteral narrowing occurred within days of percutaneous ureteral stone removal, progressing to complete occlusion in 2 cases. These complications led to prolonged hospitalization and additional procedures for each patient. One patient with an occluded ureter was lost to followup. Obstruction in the pyeloureteral region is often combined with an obstacle of an anatomical or functional character in the distally located parts of the ureter. The authors treated by operation 80 patients with congenital hydronephrosis in the period from 1986 to 1991. An additional distally located stricture of the ureter was found during intraoperative catheterization in 10 cases. In 9 of 10 cases the obstacle was at a distance of 1.7-2.0 cm from the pelvioureteral junction. Application of ureteral splint in corrective operations of the pyeloureteral segment represents still a problem to be discussed, arising diverse opinions regarding its effectiveness. A number of authors consider ureteral intubation as a necessary condition in plastic surgery of pyelone and ureter. Another group of authors reject it and consider it to be unnecessary and even dangerous, due to the possibility of it to cause additional infection and unnecessary traumas due to the splint. Pelivoureteric junction obstruction of the lower collecting system in incomplete ureteric duplication is a rare congenital anomaly. In our two cases, the connecting segment distal to the pelvioureteric junction was short and we performed direct pyeloureteral end-to-side anastomosis, without a splint catheter, with success.

In normal proximal and distal pyeloureteral human strips and in pathological reflux or obstructive segments the influence of repetitive electrical stimulation at frequencies between 0.01 and 200 Hz on their contractile behaviour was studied. Between 0.01 and 1 Hz baseline tone and maximum amplitude of contraction were dependent on stimulation frequencies, but some strips showed an irregular response pattern. At frequencies between 1 and 200 Hz half of the strips showed a typical "on" and "off" contraction (at the beginning and end of the stimulus period respectively) separated by a plateau...
