AIM: To test the effectiveness of nitinol stents in restoring patency of pyeloureteral segment (PUS). MATERIALS AND METHODS: Endoureteral nitinol stents were used in 54 patients. The indications for stenting were recurrent strictures of upper urinary tract and ureteral tumor obstruction in 34 and 20 patients, respectively. In 9 (16.6%) of them, including 4 women and 5 men aged 28-65 years, stenting was performed for extensive recurrent PUS strictures. AIM: Horseshoe kidney is often associated with other congenital abnormalities and obstruction of pyeloureteral segment (PUS). The aim of our study was to evaluate the results of laparoscopic pyeloplasty (LP) in patients with hydronephrosis of horseshoe kidney. MATERIALS AND METHODS: From February 2010 to March 2016, 130 patients underwent LP. Ten (7.7%) of them (6 men and 4 women) had a hydronephrosis of horseshoe kidney. The article presents the operation results of 380 patients (170 (44,7%) men and 210 (55,3%) women) with obstruction of pyeloureteral segment at the period from 1996 to 2014. The middle age was 43,2±7,6 years old. Primary strictures took place in 302 (79,5%) patients and recurrent strictures - in 78 (20,5%). Open and laparoscopic plastic operations were performed in 248 (65,2%), endoscopic surgery - in 112 (29,5%), organo-removal surgery - in 20 (5,3%) cases out of 380 patients with obstruction of pyeloureteral segment...

The article presents the results of laparoscopic pyeloplasty in 72 patients with primary stricture of pyeloureteral segment (PUS). The study included 34 men, 38 women; mean age was 32 +/- 4,6 years. The operation was performed by transperitoneal approach in side position of patients. 4 trocars were used. There were no conversions to open surgery. Urinary leak after surgery was observed in 4 patients; in 3 patients it has discontinued independently, and in one patient it has required a repeat laparoscopy and suturing of anastomotic defect...

We studied effectiveness of the "conflict" vessel transposition in operations for hydronephrosis due to a vasoureteral conflict in children. For 1999-2007 we operated 323 children with unilateral hydronephrosis in obstruction of the pyeloureteral segment (PUS). Ureteral endoprosthesis results are presented for 22 patients (9 males and 13 females) with long strictures of the upper urinary tract (UUT). Stenosis was caused by rectal tumor in 2 patients, by cancer of the uterine cervix in 5 patients. Examination of hydrodynamic condition of the upper urinary tracts with step-by-step pyelomanometry and profilometry of the pyeloureteral segment in 26 children at the age of 6 to 14 years with hydronephrosis provided topical diagnosis of the obstruction level. The priority role of the pelvis in development of obstructive urodynamic disorders was ascertained in 9 children. OBJECTIVES: The possible solutions to a case of ureteral duplication complicated by obstruction of one of the collecting systems are reviewed. METHODS/RESULTS: We report an uncommon case of ureteropelvic junction obstruction in the lower system of an incomplete duplication of the collecting systems that was resolved by pyeloureterostomy. In 4 patients with cutaneous urinary diversion who underwent percutaneous ureteral stone removal, similar ureteral complications developed as a result of severe ureteritis at the site of the stone. Ureteral narrowing occurred within days of percutaneous ureteral stone removal, progressing to complete occlusion in 2 cases. These complications led to prolonged hospitalization and additional procedures for each patient. One patient with an occluded ureter was lost to followup. Obstruction in the pyeloureteral region is often combined with an obstacle of an anatomical or functional character in the distally located parts of the ureter. The authors treated by operation 80 patients with congenital hydronephrosis in the period from 1986 to 1991. An additional distally located stricture of the ureter was found during intraoperative catheterization in 10 cases. In 9 of 10 cases the obstacle was at a distance of 1.7-2.0 cm from the pelvioureteral junction. Application of ureteral splint in corrective operations of the pyeloureteral segment represents still a problem to be discussed, arising diverse opinions regarding its effectiveness. A number of authors consider ureteral intubation as a necessary condition in plastic surgery of pyelone and ureter. Another group of authors reject it and consider it to be unnecessary and even dangerous, due to the possibility of it to cause additional infection and unnecessary traumas due to the splint.
