Renal involvement of systemic vasculitides is an emergency in nephrology. Although it has become very rare, the diagnosis of polyarteritis nodosa must be considered in some patients. A 70-year-old patient, previously healthy, presented with acute renal failure and malignant hypertension and abundant albuminuria. Subcutaneous nodule, orchitis and mononeuritis appeared subsequently. The search for auto-immunity or viral infection was negative. Markers of thrombotic microangiopathy, present initially, resolved after blood pressure control...

This review addresses the pathophysiology of hemorrhagic shock, a condition produced by rapid and significant loss of intravascular volume, which may lead to hemodynamic instability, decreases in oxygen delivery, decreased tissue perfusion, cellular hypoxia, organ damage, and death. The initial neuroendocrine response is mainly a sympathetic activation. Haemorrhagic shock is associated altered microcirculatory permeability and visceral injury. It is also responsible for a complex inflammatory response associated with hemostasis alteration...

Oxygen adiministration for both medical and traumatic emergencies is regarded as an essential component of resuscitation. However, many recent studies suggest that the use of oxygen should be more restrictive. Detrimental effects of normobaric oxygen therapy in patients suffering from hypercapnic respiratory diseases have been demonstrated, especially because of the suppression of the hypoxic drive. Apart from this particular situation, correction of hypoxemia is still a widely accepted treatment target, although there is growing evidence that hyperoxemia could be harmful in acute coronary syndromes and cardio-respiratory arrests...

An 80-year-old man presented to our hospital with macroscopic hematuria. Before coming to our hospital, he was treated for multiple myeloma with cyclophosphamide. We detected diffuse dilation of capillary on urinary bladder mucosa on cystoscopy, and diagnosed the patient with cyclophosphamide-induced hemorrhagic cystitis. Macroscopic hematuria once resolved by indwelling urethral catheter, but recurred and bladder tamponade developed. We performed transurethral electric coagulation. Macroscopic hematuria temporarily disappeared, but again relapsed...

Dengue is a common, mosquito borne, travel-related, viral infection in the tropical traveler caused by an arbovirus. This virus has 4 different serotypes and is responsible for approximately 50 to 100 million cases per year. Its incidence and geographic distribution are clearly increasing lately. Although occurring in tropical endemic areas, sporadic and autochthonous cases have been recently reported in temperate countries including France. The tropical influenza-like clinical presentation is not very specific except for rash...

BACKGROUND AND OBJECTIVES: Dextran associated with 7.5% sodium chloride has positive hemodynamic effects in the long term control of hemorrhagic shock. The objective of this study was to verify whether the association of dextran with 7.5% chloride solution would be advantageous in the immediate hemodynamic evaluation of controlled hemorrhagic shock in dogs. METHODS: This study included 16 dogs submitted to controlled hemorrhage until their mean arterial blood pressure reached 40 mmHg, being maintained at this level for 30 minutes...

The purpose of this study was to assess the feasibility and effectiveness of percutaneous endovascular repair of ruptured abdominal aortic aneurysms (AAAs) previously treated by EVAR. In the last year, two male patients with AAAs, treated 8 and 23 months ago with bifurcated stent-graft, were observed because of lumbar pain and hemorragic shock. Multidetector computed tomography (MDCT) showed a retroperitoneal hematoma; in both cases a type III endoleak was detected, in one case associated with a type II endoleak from the iliolumbar artery...

REVIEW

This review addresses the pathophysiology and treatment of hemorrhagic shock, a condition produced by rapid and significant loss of intravascular volume. Hemorrhagic shock may lead sequentially to hemodynamic instability, decreases in oxygen delivery, decreased tissue perfusion and cellular hypoxia. Multiple organ failure, a systemic inflammatory process that leads to dysfunction of different vital organs, is a frequent complication after hemorrhagic shock and accounts for a high incidence of mortality. The pathogenesis of organ injury secondary to hypovolemic insults is still not completely understood, but both experimental studies and clinical observations indicate that macrophages are activated by translocated endotoxin-bacteria and ischemia/reperfusion...

Inferior vena-cava (IVC) injuries are uncommon but mortality is high, despite the improved organization and technical skills. The most important prognostic factors are the grade of hemorragic shock, the anatomical level of caval injury and the associated vascular lesions. Supra and retro-hepatic lacerations have the worst prognosis and their management is more difficult, as it's sometimes necessary performing complicated and high mortality related procedures, such as atrio-caval shunt and total vascular occlusion...

Most patients with trauma-hemorragic shock, prior to ICU admission, have been resuscitated and stabilized in the emergency room (ER) and/or operation room (OR). Many of them suffer from systemic edema. This extra-vascular fluid is caused by massive infusion of fluid and blood for the maintenance of blood pressure. During the recovery stage, the patients exhibit spontaneous diuresis followed by negative fluid balance. Urine volumes of some patients are more than 10000 ml/d. Do we need to maintain a balance between daily input and output of water at this situation? There are many references in the medical literature and textbooks about fluid resuscitation and the principles in maintaining the balance between input and output of water, but rarely about when and how to restrict it, that is, when and how to permit a negative balance...

A single exposure to intense impulse noise may cause diffuse brain injury, revealed by increased expression of immediate early gene products, transiently altered distribution of neurofilaments, accumulation of beta-amyloid precursor protein, apoptosis, and gliosis. Neither hemorrage nor any gross structural damage are seen. The present study focused on whether impulse noise exposure increased the permeability of nerve and glial cell membranes to proteins. Also, we investigated whether a preceding, minor focal surgical brain lesion accentuated the leakage of cytosolic proteins...

Blood loss leads to the reduction in vitality of red blood cells (RBCs). However, the changes in morphology at different stages of hemorrhagic shock have not been studied. Thus, the aim of this study was to identify and quantitate the sequence of morphological changes in RBCs during hemorrhage. This study was performed on 15 adult inbred dogs. Blood samples were taken before hemorrhage, when the mean arterial pressure reached 40 mm Hg (initial stage of shock), and at a mean arterial pressure level of 20 mm Hg (decompensated stage of shock)...

In experiments on dogs, which were anesthetized and underwent hemorragic shock, the hemodynamics, oxygen transport and acid-base status were studied. Cardiac function was studied using ultrasound equipment. It was established that administration of Volekam and Polyoxidin corrects hypovolemia with subsequent restoration of arterial pressure in the aorta and cardiac cavities and increase in the myocard contractility and cardiac output. Comparative analysis has shown that Polyoxidin exerts a more prolonged action than Volekam...

The effects of Dextran-Benzene-Tetracarboxylate-Hemoglobin (Dex-BTC-Hb), a chemically-modified hemoglobin-based oxygen carrier, on the vascular tone were compared to those of standard solutions, i.e. the animal's own blood and a 50 milligrams albumin solution, by measuring the carotid blood flow velocity, the mean arterial pressure, the heart rate and respiratory frequency, in anesthetized Hartley guinea pigs after a hemorragic shock. Stroma-free hemoglobin induced 40% hypertension and a 110% rise in blood flow velocity immediately after injection...

The child with a severe head injury requires a three component's treatment: against cerebral edema, against restlessness and vegetative trouble, against shock in case of scalp-hemorrage or attendant traumatism. As there is no agreement neither about the treatment of cerebral edema nor that of sedative treatment, we continue to use systematically artificial ventilation, sometimes corticoids, rarely mannitol. Decerebration is treated by a lytic coktail enclosing always diazepam and in case of failure, sodium gamma-hydroxybutyrate...

The authors discuss the problems of emergency treatment of cavernous hemangiomas of the liver. Five cases were observed and treated with different techniques, ligation of the hepatic artery, excision of the mass, embolization of the hepatic artery. The results of these procedures were strongly influenced by the patient's previous state. Treatment was successful in three patients, while the procedure adopted was able to stop the hemorrage in the other two patients. The physical state was very important for the prognosis; the two patients arriving at our Institute in deep shock both died...
