The 12-lead ECG is a fundamental modality to help determine the mechanism and the localization of atrial tachycardias (ATs). Although macroreentrant atrial tachycardias (ATs) and focal ATs typically show F-waves and discrete P-waves respectively on the 12-lead ECG, this is not universally the case in scar-related ATs.1,2 We present three cases clearly showing the discrepancy between the AT morphology on the 12-lead ECG and the AT-mechanism.
