This case presentation is about an 88 years-old male patient with previous endovascular aortic aneurysm repairment history and aortic endoleak type II (EL2). The direct lumbar artery catheterization was considered an alternative to solve EL2, associated with aortic endovascular prosthesis and due to an incomplete sealing or exclusion of the aneurysmal sac or a vascular segment demonstrated by imaging studies, when other treatment alternative failed (transarterial embolization) to control the aneurysm growing...

INTRODUCTION: Previous studies have suggested that there is a positive correlation between the extent of video gaming and efficiency of surgical skill acquisition on laparoscopic and endovascular surgical simulators amongst trainees. However, the link between video gaming and orthopaedic trauma simulation remains unexamined, in particular dynamic hip screw (DHS) stimulation. OBJECTIVE: To assess effect of prior video gaming experience on virtual-reality (VR) haptic-enabled DHS simulator performance...

PURPOSE: Exposure to ionizing radiation during electrophysiologic procedures in children is believed to increase the risk of future malignancy. Electroanatomical navigation can reduce exposure, but the cohort of children who derive the greatest benefit from this approach is incompletely defined. We sought to determine factors associated with fluoroscopy exposure with conventional catheter ablation versus electroanatomical navigation. METHODS: A retrospective review of all ablation procedures over a 5-year period during the transition to electroanatomical navigation performed by a single electrophysiologist was performed...

Our patient had recurrent syncope due to ventricular tachycardia (VT) after one year of VVI Pacemaker implantation. He had pacemaker pocket infection for which new pacemaker was implanted on opposite side but old lead was not explanted completely. Flouroscopy showed redundant loop of old lead in right ventricular inflow which was snared out subsequently. He never had syncope or VT after that.

BACKGROUND: In this study we aimed to evaluate the long-term outcome of diaphragmatic plication for symptomatic unilateral diaphragm paralysis. METHODS: Thirteen patients who underwent unilateral diaphragmatic plication (2 patients had right, 11 left plication) between January 2003 and December 2006 were evaluated. One patient died postoperatively due to sepsis. The remaining 12 patients [9 males, 3 females; mean age 60 (36-66) years] were reevaluated with chest radiography, flouroscopy or ultrasonography, pulmonary function tests, computed tomography (CT) or magnetic resonance imaging (MRI), and the MRC/ATS dyspnea score at an average of 5...

During prostate brachytherapy, C-arm flouroscopy images are used for a qualitative assessment of the procedure. Three dimensional reconstruction of the implanted seeds can be used for intraoperative dosimetry and quantitative assessment. Accurate C-arm pose estimation is necessary for 3D localization of the seeds. We propose to measure the C-arm rotation angles and computationally compensate the inevitable C-arm translational motion to estimate the pose. We compensate the oscillation, sagging and wheel motion of the C-arm using a three-level optimization algorithm, without which the reconstruction can fail...

Computer-aided surgery (CAS) allows for real-time intraoperative feedback resulting in increased accuracy, while reducing intraoperative radiation. CAS is especially useful for the treatment of certain pelvic ring fractures, which necessitate the precise placement of screws. Flouroscopy-based CAS modules have been developed for many orthopedic applications. The integration of the isocentric flouroscope even enables navigation using intraoperatively acquired three-dimensional (3D) data, though the scan volume and imaging quality are limited...

Most interventional pain physicians are experts in the use of fluoroscopy and understand its importance in improving the safety, accuracy and efficacy of diagnostic and therapeutic procedures. The addition of digital subtraction may enhance the utility of radiography and improve the accuracy of our interpretations. This article illustrates the use of the technique in several patients. The utility of this enhanced technique in clinical versus research application remains to be determined.

The aim of this pilot study was to evaluate whether the technique of magnetic fluid hyperthermia can be used for minimally invasive treatment of prostate cancer. This paper presents the first clinical application of interstitial hyperthermia using magnetic nanoparticles in locally recurrent prostate cancer. Treatment planning was carried out using computerized tomography (CT) of the prostate. Based on the individual anatomy of the prostate and the estimated specific absorption rate (SAR) of magnetic fluids in prostatic tissue, the number and position of magnetic fluid depots required for sufficient heat deposition was calculated while rectum and urethra were spared...

UNLABELLED: A pseudo r' in V1 during supraventricular tachycardia (SVT), but not during sinus rhythm is pathognomonic for AV nodal re-entry tachycardia (AVNRT). During radiofrequency (RF) energy delivery, stability of the catheter tip is crucial. Intra-procedural catheter and patient movement as well as abrupt rhythm changes can lower efficacy, prolong procedural time, and contribute to the risk of AV block. OBJECTIVES: A novel streamlined approach using a single sheath and two catheters was evaluated that leverages the patient's own anatomy to help stabilize catheter position during RF application and localize RF targets...

Four cases of severe mitral regurgitation due to disc variance of the Harken disc prosthesis in the mitral position are described. The valve occluder actually escaped into the left atrium in two patients, and neither survived despite emergency valve replacement. In the other two, disc malfunction was identified by flouroscopy, the prosthesis was replaced, and both patients survived. All four patients had associated aortic regurgitation, which most likely contributed to erosion of the disc edges. It is suggested that patients with the Harken disc prosthesis undergo periodic evaluation to detect abnormal disc motion...

In this paper we present a trainings system for catheter interventions, especially PCTAs also called ballon dilatation. Despite the large number of such interventions the education of the cardiologist is still based on learning on the patient. This means risk to the patient and additional cost. To overcome these problems we developed the CathI system (Catheter Instruction System), a realistic computerbased training system. It provides the physician with the same equipment found in a catheter laboratory, e.g...
