We describe the diagnosis, treatment, rehabilitation, and complications of a 27-year-old male paraskiier who sustained a closed high-velocity knee dislocation after colliding with a tree at a speed of 45 mph and falling 40 feet to the ground. Extensive surgery to repair and reconstruct the knee included open lateral capsular repair, open anatomical posterolateral corner reconstruction with split Achilles tendon allograft, open iliotibial band reconstruction with Achilles tendon allograft, arthroscopic anterior cruciate ligament reconstruction with bone-patellar tendon-bone autograft, and double-bundle posterior cruciate ligament reconstruction with Achilles tendon allograft for the anterolateral bundle and tibialis anterior tendon allograft for the posteromedial bundle...

Magnetic resonance imaging and a thorough understanding of its interpretation in the diagnosis and management of injuries to the ligaments about the knee is an essential skill for orthopedists. The goal of this review is to provide a description of the native and injured anatomy through magnetic resonance imaging and its correlations, when appropriate, as seen through the arthroscope at the time of surgery. The ligaments of focus include the anterior cruciate ligament, the posterior cruciate ligament, the medial collateral ligament, the posterolateral corner, the anterolateral ligament, and the medial patellofemoral ligament...

BACKGROUND: Two popular physeal-sparing procedures used in the management of anterior cruciate ligament (ACL) injuries in skeletally immature patients are the iliotibial band (ITB) ACL reconstruction (ACLR) and the all-epiphyseal (AE) ACLR. Although there has been concern for overconstraint of the lateral compartment of the knee with the ITB ACLR technique, rotational stability, as provided by the anterolateral ligament (ALL) and ACL, has not been assessed in the setting of pediatric ACLR techniques...

INTRODUCTION: During anterior cruciate ligament (ACL) reconstruction procedures, anterolateral reconstruction (ALR) can also be performed to improve the knee's rotational stability. However, the effectiveness of this supplemental technique and its impact on the risk of retears and on the onset of secondary degenerative changesare controversial. HYPOTHESIS: ALR improves control over the pivot shift, reduces the retear risk and delays the appearance of secondary degenerative lesions...

In this article we provide an overview of impingement syndromes of the lower limb. At the level of the hip, femoroacetabular and ischiofemoral impingement are recognised. At the level of the knee, we discuss Hoffa's fat pad impingement, suprapatellar fat pad impingement, pericruciate impingement, and iliotibial band syndrome. The impingement syndromes associated with anterior cruciate ligament (ACL) repair and intercondylar osteophytes are also illustrated. Most impingement syndromes are described at the level of the ankle...

BACKGROUND: The purpose of this study was to define the best anatomic parameters with which to perform an accurate anterolateral ligament (ALL) reconstruction. These parameters were anatomical insertions, allowing favorable isometry, length variation during flexion, and anthropometric predictors of ALL lengths. METHODS: A total of 84 fresh-frozen cadaver knees were dissected to analyze the ALL, focusing on its femoral insertion. The ALL length was measured in different degrees of flexion (extension, 30°, 60°, and 90° of flexion) and rotation (neutral, internal or external rotation)...

The role of the anterolateral capsule complex in knee rotatory stability remains controversial. Therefore, the objective of this study was to determine the in situ forces in the anterior cruciate ligament (ACL), the anterolateral capsule, the lateral collateral ligament (LCL), and the forces transmitted between each region of the anterolateral capsule in response to a simulated pivot shift test. A robotic testing system applied a simulated pivot shift test continuously from full extension to 90° of flexion to intact cadaveric knees (n = 7)...

A recent systematic review on the techniques, biomechanics, and clinical outcomes of anterolateral ligament reconstruction of the knee summarizes our current understanding of this hotly debated topic. In 2017, it is not clear if an anterolateral ligament reconstruction is needed in the setting of primary anterior cruciate ligament reconstruction.

BACKGROUND: Anterolateral tenodeses are increasingly popular in combination with intra-articular anterior cruciate ligament reconstructions. Despite the perception of risk of overconstraint and lateral osteoarthritis, evidence is lacking regarding the effect of graft tensioning on knee kinematics and intra-articular compartmental joint pressures. PURPOSE: To investigate tibiofemoral joint contact pressures and kinematics related to an anterolateral lesion and the effectiveness of a MacIntosh tenodesis in restoring these when varying (1) graft tension and (2) tibial rotation during graft fixation...

BACKGROUND: Segond's fracture is a well-recognised radiological sign of an anterior cruciate ligament (ACL) tear. While previous studies evaluated the role of the anterolateral ligament (ALL) and complex injuries on rotational stability of the knee, there are no studies on the biomechanical effect of Segond's fracture in an ACL deficient knee. The aim of this study was to evaluate the effect of a Segond's fracture on knee rotation stability as evaluated by a navigation system in an ACL deficient knee...
