PURPOSE: To describe the refractive outcomes of triple Descemet membrane endothelial keratoplasty (DMEK). DESIGN: Retrospective case series. METHODS: The study included patients with Fuchs endothelial dystrophy and cataract without coincident pathology. RESULTS: Outcomes of 108 sequential triple DMEK procedures were evaluated. With a mean follow-up of 11.9 months, the median corrected distance visual acuity (CDVA) was 20/20 (range 20/15 to 20/40) and the median uncorrected distance visual acuity in eyes with a distance target (n = 84) was 20/40 (range 20/20 to 20/200)...

BACKGROUND AND PURPOSE: Descemet stripping automated endothelial keratoplasty (DSAEK) is an established treatment option for pathologies of the corneal endothelium. It is typically accompanied with a hyperopic shift in refraction. The purpose of this work is to predict corneal geometry after DSAEK based on model data and to present a concept how to determine corneal power, e.g. for intraocular power calculation to prevent a refractive surprise with a subsequent cataract surgery. MATERIAL AND METHODS: Based on data of the Kooijman schematic model eye we simulated a microkeratome cut parallel to the corneal front surface for donor trephination to determine the radial thickness profile of the posterior corneal donor lamella...

Although there are many formulae for the calculation of intraocular lens power in the eyes with previous kerato-refractive surgeries, unexpected refractive bias still exists. Hyperopic bias is particularly disliked because it affects both uncorrected distance and near visual acuity. Surgical treatment of the residual hyperopia for the eyes with both laser in situ keratomileusis and cataract surgery remains to be a big problem. Conductive keratoplasty has been shown to be an effective, safe and predictable method for low and moderate hyperopia in the pseudophakic eyes or in the eyes with kerato-refractive surgeries...

PURPOSE: To evaluate change in graft steepness after graft refractive surgery (GRS) consisting of relaxing incisions with or without counterquadrant compression sutures and discover the existing influential factors. METHODS: In this retrospective study, 78 eyes of 76 patients who had received penetrating keratoplasty for keratoconus underwent GRS because of high post-penetrating keratoplasty astigmatism. Any shift in graft curvature was calculated using the keratometric coupling ratio (CR; the ratio of flattening of the incised meridian to steepening of the opposite meridian)...

INTRODUCTION: Simultaneous Descemet stripping endothelial keratoplasty, phacoemulsification, and intraocular lens implantation are indicated in Fuchs' dystrophy with associated cataract. Compared to the standard method of the triple procedure which includes penetrating keratoplasty, this new method has the advantages of sutureless surgery, small limbal incision, faster recovery, less surface problems, less astigmatism, stronger tensile strength and more predictable calculation of the intraocular lens power...

BACKGROUND: A high corneal astigmatism limits the attainable visual acuity and the ability of correction with spectacles or contact lenses. The astigmatism can also be an impairment for visual rehabilitation, especially after penetrating keratoplasty. A pseudohakic toric intraocular lens (T-IOL) can correct high corneal astigmatism during cataract surgery. For the calculation of the correct power for the T-IOL several additional factors need to be considered, which are less relevant for spherical IOL calculation...

BACKGROUND: We report the results of the secondary implantation of retroiridally fixated irisclaw-lenses (Artisan(R)) to correct aphakia. PATIENTS AND METHODS: This retrospective study included all eyes that had undergone outpatient implantation of an Artisan IOL by the senior author (MK) between July 2004 and January 2008. We analysed 48 eyes of 46 patients (27 female, 19 male) aged 17 to 87 years. Underlying conditions were: aphakia after congenital cataract or trauma (19 eyes), late luxation of a posterior chamber IOL (16 eyes), intraoperative zonulolysis during cataract surgery that did not allow PC IOL implantation (12 eyes, including 10 eyes with pseudoexfoliation and one eye with Marfan syndrome), anterior chamber IOL with corneal decompensation problems and recurrent hyphema (1 eye)...

PURPOSE: To report intraocular lens (IOL) power calculation in 2 eyes that were highly undercorrected by previous myopic automated lamellar keratoplasty (ALK). METHODS: A 35-year-old man underwent bilateral myopic ALK, which caused high residual myopia (-9.0 -4.0 x 171 and -9.5 -4.5 x 74). The patient then underwent cataract surgery with IOL implantation for cataract development. The double-K clinical history method was utilized, and satisfactory IOL power prediction results were obtained...

The importance of quality of vision (QOV) along with quality of life (QOL) in medicine has been recently widely recognized. We have conducted studies to quantitatively analyze factors related to QOV. Irregular astigmatism can be a significant obstacle for achieving satisfactory QOV. Videokeratography data were broken down using Fourier harmonic series analysis into spherical power, regular astigmatism (second harmonic component, n = 2), asymmetry (n = 1), and higher order irregularity (n > or = 3). The irregular astigmatism component calculated by the Fourier analysis significantly correlated with best spectacle-corrected visual acuity...

Corneal diseases subject to keratoplasty are often combined with a certain degree of cataract. Therefore, appropriate strategies for the management of cataract are mandatory. One strategy is a sequential procedure performing cataract surgery after keratoplasty. Though this procedure enables us to estimate IOL power accurately, there are several drawbacks including multiple procedures, slow visual rehabilitation and endothelial damage during cataract surgery. On the other hand, triple procedure, consisting of penetrating keratoplasty, extracapsular lens extraction and IOL implantation, has several advantages, such as single procedure, rapid visual rehabilitation and no additional endothelial trauma...
