The TREAT EARLIER was aimed at testing whether methotrexate could prevent the evolution from clinically suspect arthralgia to Rheumatoid Arthritis. Glucocorticoids are used in the treatment of rheumatoid arthritis since many decades. Glucocorticoids are proven effective in decreasing inflammatory activity of this disease and in retarding the progression of erosive joint damage. However, they also have multiple possible adverse effects, which are depending on dose and duration of treatment. Due to increasing treatment options (i.e. biologic and targeted synthetic disease modifying antirheumatic drugs) the use of glucocorticoids is subject of debate. Tofacitinib is a relatively new oral drug for the treatment of rheumatoid arthritis. The ORAL surveillance study shows in a direct comparison that more patients with RA develop a 'major adverse cardiovascular event' (MACE) or malignancy during treatment with tofacitinib than during treatment with a tumor necrosis factor (TNF) inhibitor. The reliability of the study, the generalisability and interpretation of the results, as well as their implication for daily practice are discussed here.
