Some patients receive multiple appropriate shocks during a short period of time, arrhythmic electrical storm (malignant ventricular arrhythmias resulting in device intervention > or = 3 times during 24 hours). It is a common implantable cardioverter-defibrillator therapy-related complication. The objectives of this study were to determine the incidence, characteristics, prognostic implications and therapeutic options of electrical storm. This six years prospective study comprised 115 device recipients, who were followed for 510-2,100 days...

OBJECTIVE: The study analyzed the role of different variables that determine long-term sinus rhythm maintenance in patients with persistent atrial fibrillation who are treated with amiodarone. BACKGROUND: It has been recognized that different factors influence long-term sinus rhythm preservation after the conversion of persistent atrial fibrillation. Although the duration of the arrhythmia appears to be the most important factor, consistent information regarding the role of the mode of arrhythmia conversion (pharmacologic or electric) is still lacking...

Amiodarone has been used as an anti-arrhythmic drug since the 1970s and has an established role in the treatment of ventricular tachyarrhythmias. Although considered to be a class III anti-arrhythmic, amiodarone also has class I, II and IV actions, which gives it a unique pharmacological and anti-arrhythmic profile. Amiodarone is a structural analogue of thyroid hormone and some of its anti-arrhythmic properties and toxicity may be attributable to interactions with nuclear thyroid hormone receptors. The lipid solubility of amiodarone gives it an exceptionally long half-life...

From may 1986 to december 1991, we have performed 90 heart transplantation in the Hospital "Reina Sofía" from Córdoba. In the present study we analyze the early morbidity and mortality in a retrospective study on complications and factors determining its incidence. Seventy patients were male (86%) and 13 female (14%). Age ranged from 12 and 65 years. The 31% of patients needed in pharmacological support before transplantation, 15 of them were operated in a emergency situation because of cardiogenic shock...

We present the case of a 10-year-old boy, with structurally normal heart who began two years before admission, with sustained monomorphic ventricular tachycardia with heart rate of 280 bpm and LBBB morphology, AQRS + 60 degrees, with pallor and diaphoresis. The tachycardia was treated with xylocain. He was also treated with propaphenone, verapamil, and amiodarona, in spite of these, he continued with this type of arrhythmia 2 or 3 times a month. We performed an electrophysiologic study which showed ventricular tachycardia originated in the right ventricle infundibulum...
