The Stockholm-Gotland Cancer Register was used to study the risk of developing second primary malignancies (SPM) in women diagnosed with cancer of the uterine cervix, uterine corpus and ovaries during the period 1958-1992. Among 5,325 patients with uterine cervix cancer, 619 developed SPM. Standardized incidence ratio (SIR) was 1.29 (95% confidence interval (CI) 1.19-1.39). 7054 malign neoplasms were observed at the I. Univ.-Frauenklinik of Vienna in the years 1950-1969. 142 cases were multiple primary neoplasms, i.e. 2.01%. In 51 cases the double neoplasms were situated in the genital region (local multiplicity), in 91 cases was there a separated multiplicity with the cancer of the breast dominating. In 151 patients the plasma-ribonuclease-activity was measured. The results of a control group of 47 patients were compared to the pretherapeutic values of 21 cases with cervical intraepithelial neoplasia, 83 cases with gynecologic cancer and 47 cases with breast cancer. The upper limit of the normal range was 300 ng/ml. The method is not appropriate for the detection of precancer of the uterine cervix or for breast cancer. For gynecologic tumors we found a sensitivity rate of 87% and a specificity rate of 85%...

The increasing life expectancy of our population will mean that by the year 2000 we may expect a large number of women to be in age groups at significant risk for malignancies of the female reproductive tract. It is very unlikely that we will soon understand the cause of cancer, and effective new therapies are not to be anticipated. Rather, improved survival and diminished morbidity must be obtained by insuring ready and equal access to state of the art treatment, for the patient and physician.
