enfermedad devic

Louise L Lambrichs, Chantal Marazia, Luis Montiel
In the birth and development of the Medical Humanities, literature has played a crucial role, both as an educational resource and as an analytical device. This article proposes an appraisal of this approach by focussing on a model literary situation. Taking the move from Alberto Barrera Tyszka's novel La enfermedad [The illness], the authors identify and explore some of the "places of ignorance" that can emerge in the doctor-patient relationship, as potentially in any form of intersubjective encounter. Invasive medical technology has led to an increase in the incidence of healthcare-associated infective endocarditis (HAIE). A prospective multicentre cohort study was conducted at seven hospitals in Andalusia, Spain, to establish the characteristics of HAIE and to compare them with those of community-acquired infective endocarditis (CAIE). HAIE was defined as either infective endocarditis (IE) manifesting >48 h after admission to hospital, or IE associated with a significant invasive procedure performed in the 6 months before diagnosis...

Bacteremia and endocarditis due to methicillin-resistant Staphylococcus aureus (MRSA) are prevalent and clinically important. The rise in MRSA bacteremia and endocarditis is related with the increasing use of venous catheters and other vascular procedures. Glycopeptides have been the reference drugs for treating these infections. Unfortunately their activity is not completely satisfactory, particularly against MRSA strains with MICs > 1 microg/mL. The development of new antibiotics, such as linezolid and daptomycin, and the promise of future compounds (dalvabancin, ceftobiprole and telavancin) may change the expectatives in this field...

OBJECTIVE: The objectives of this study are: 1) to determine the improvement in the cardiovascular health of the population after the introduction of the clinical governance contract for primary care team professionals in Tarragona-Reus and the Terres de l'Ebre area (Tarragona province, Spain); 2) to identify the factors predictive of better cardiovascular health after the introduction of the clinical governance contract. The introduction of the clinical governance contract, which is based on professional leadership, feed-back of care information, and monitoring of indicators of cardiovascular risk based on scientific evidence and concretised in clinical practice guidelines, will improve the cardiovascular health results of the reference population...

OBJECTIVES: To establish reference values for blood pressure by means of self-measurement of blood pressure (BP) conducted at home. DESIGN: Descriptive study of the distribution of self-measured BP at home and its correspondence with clinic-based measurements of BP. METHODS: The aim of this study is to define the home BP levels that correspond to clinic BP thresholds 140/90 mmHg (hypertension) and 130/85 mmHg (normality). The sample consisting of 1411 randomly selected adults stratified by age and gender...
