The combination of an ipsilateral clavicule and scapula fracture causes a double instability of the shoulder. In opposite to an isolated clavicule or scapula fracture this injury should be treated operatively. Diagnosis of the scapula fracture in polytrauma can be difficult. If there is a clinical or radiologic suspicion, the indication for a CT scan is given. Osteosynthesis with plates from separated approaches, first the scapula from dorsal then the calvicule from ventral has proven good. Obstetrical fracture has not a bad character, because of quick healing and, above all, because of extraordinary modelling by growth. The authors record 33 fractures of long bones which confirm this good prognosis. 20 birth fractures of the clavicule associated with brachial plexus palsies are recorded on account of their pathogenic, diagnostic and therapeutic intricacies. A case of a 71-year-old male patient with a branchiogenetic cervical cyst is presented. In addition to the clinical manifestation in the eight decade the speciality of this cyst was epipleural localisation. The longest diameter of the cyst was parallel to the right clavicule under the M. omohyoid. The cyst medially filled up the venous angle. Embryology, diagnosis, differential diagnosis as well as the therapy and prognosis are discussed in regard of cases of literature.
