Transvaginal hydrolaparoscopy (THL) is considered as a first-line endoscopic method in the investigation of infertility. During the last period it has played important role in the rehabilitation of the natural fertility. The rapid development of this method in the operative settings is very attractive by it's low-invasiveness. Coagulation of endometrial cysts using THL method is very effective. In this case operation can be performed on an outpatient basis. The detection of small endometriomas that are not seen during THL is not a problem only for fertiloscopy...

OBJECTIVE: To investigate the advantages of fertiloscopy in the examination and therapy of infertile women. METHODS: One hundred and fifteen infertile patients underwent fertiloscopy including transvaginal hydrolaparoscopy (THL), conventional dye-test, hysteroscopy, and dye-test using catheterization of the tubal ostium by hysteroscopy from May 2003 to Mar 2005. Access to the pouch of Douglas was achieved in 110 patients (95.7%). The primary infertile patients (primary group) and secondary infertile patients (secondary group) included respectively 49 and 61 cases...

The first line treatment of the polycystic ovary syndrome is a medical one. It begins with dietary-induced weight loss, dyslipidemy treatment or insulin-sensitising agents. In the case of infertility, as primary intention, the patients are treated by citrate of clomiphene. In case of failure, the indications of laparoscopy have to be set up as an alternative to utilization of gonadotrophin and recombinant FSH associated to intra-uterine insemination. According to the results of the meta-analysis of Campo, medical and surgical approaches give an identical rate of pregnancy (40%) but can reach 48-50% in the surgical group after one year because the pregnancy rate continues to grow, when the women treated medically do not have a pregnancy in the absence of treatment...

Two complications of ovarian drilling by fertiloscopy are described about two patients with polycystic ovary syndrome. Ovarian drilling by fertiloscopy is a new minimally invasive surgery, but this procedure is not without risks. It requires larger studies with longer follow-up to evaluate effectiveness and risks.

Hysterosalpingo-contrast-sonography (HyCoSy) using saline and Echovist is a well tolerated outpatient technique that provides a significant amount of information of relevance to the infertile woman that is not obtainable at hysterosalpingogram (HSG) whilst avoiding exposure to X-ray irradiation. When performed by experienced operators, it serves as a valuable, first-line screening test for the more invasive procedures of laparoscopy and dye chromopertubation and hysteroscopy. If detailed diagnostic information is required in women in whom there is no clinical or ultrasound evidence of pelvic pathology, the surgical technique of fertiloscopy can be considered to be appropriate...

BACKGROUND: The aim of this prospective multicentre study was to compare the two endoscopic techniques of laparoscopy and fertiloscopy in routine evaluation of the pelvis in infertile women. METHODS: A total of 92 women was selected in 14 University Hospitals to undergo fertiloscopy followed by transabdominal laparoscopy by a team of two surgeons in each hospital. RESULTS: A high degree of concordance was observed between these two techniques, in that if fertiloscopy did not detect any abnormalities, this was also confirmed by laparoscopy...

Fertiloscopy is a useful first level alternative procedure in order to diagnose infertility. It is a new diagnostic technique, which combines culdoscopy, dye-test, salpingoscopy and hysteroscopy and uses a specific disposable kit (Fertiloscopy Soprane). Fertiloscopy has the advantage of limiting pelviscopy only when strictly indicated, while maintaining a high diagnostic quality. Furthermore, it is more cost-effective by reducing the use of the operating theatre, the number of doctors and nurses involved and also by decreasing surgical risks and therefore leading to a better compliance by the patients...

We have defined fertiloscopy as the combination in one investigation of transvaginal hydropelviscopy, dye-test, optional salpingoscopy, and hysteroscopy, performed on an outpatient basis under local anaesthesia or neuroleptanalgesia. We have applied this approach in a routine manner to 160 infertile patients with no obvious pathology. Fertiloscopy was achieved in 154 patients (96.2%). In five patients visualization was not satisfactory because of technical problem or adhesions in the pouch of Douglas. We had one (0...
