BACKGROUND: Differentiation of vertebral metastases from Schmorl's nodes, especially with fluorine-18-fluorodeoxyglucose ((18)F-FDG) uptake, is very important for the appropriate management of these patients in case they have malignancy. We aimed to evaluate the value of (18)F-FDG positron emission tomography/computed tomography (PET/CT) in differentiating vertebral metastases from Schmorl's nodes with (18)F-FDG uptake. SUBJECTS AND METHODS: Fourteen patients with malignancy underwent (18)F-FDG PET/CT, which showed abnormal (18)F-FDG uptake in 19 Schmorl's nodes...

UNLABELLED: In the bordeline personality disorder a large role ascribe to biopsychosocial factors. Studies have shown that more than 70% patients BPD reported experiencing traumatic events in childhood. The findings are confirming that making self-harming is a frequent symptom of bordeline disorder and 70-75% patients show at least one act of self-harming. Selfharming can be a reaction to maladaptive emotional regulation. Moreover a lowered tolerance level is characteristic of them to the stress and determined course learning dysfunctional patterns of behavior...

The authors critically analyse the tendency of conceptualizing some difficult to classify psycho-pathological states, the symptoms and clinical dynamics of which would be characteristic, in a psycho-analytical interpretation, to a nosologically distinct category termed "bordeline states". The authors' orientation and clinical observations related to the data in the literature lead to the specification of those situations in which this term can be used in the psychiatric diagnosis, preferably under its original lexical form and with the specification of the substantiating concept...

Background. Levosimendan is an inotropic drug with unique pharmacological advantages in patients with acute heart failure. Scope of this study is to determine whether longer infusion patterns without the hypotension-inducing loading dose could justify an effective and safe alternative approach. Methods. 70 patients admitted to the emergencies with decompensated chronic heart failure received intravenously levosimendan without a loading dose up to 72 hours. Clinical parameters, BNP (Brain Natriuretic Peptide) and signal-averaged-ECG data (SAECG) were recorded up to 72 hours...

BACKGROUND: There is a significant relationship between obstructive sleep apnea (OSA) and cardiovascular diseases. Reliability of new methods evaluating apnea in Holter ECG monitoring is still the matter of investigators' studies. METHODS: In 48-hour Holter ECG monitoring recordings of 63 patients, we assessed repeatability, comparing the results from both sleep periods. RESULTS: We found good repeatability in evaluation of apnea-hypopnea index value...

BACKGROUND AND AIMS: Arterial hypertension is a well known risk factor for cardiovascular diseases. Today, it is possible to calculate the cardiovascular risk at 10 years with the risk card. The reduction of cardiovascular risk is based on a multi-factorial approach including the lifestyle modification. In Italy, OEC study has calculated that a certain proportion of borderline hypertensives, not eligible for a pharmacological treatment, remain at risk. Borderline arterial hypertension (140-150/90-95 mmHg) in Italian population is documented in 19% of males and 14% of females...

Self-injurious and suicidal behaviours are highly prevalent in patients with borderline personality disorder (BPD) and the risk of completed suicide is high. Borderline patients often present with heterogeneous clinical pictures and widespread comorbidity complicating clinical assessments and management. This calls for increased efforts in systematic evaluation and monitoring of self-harming and suicidal behaviours; these behaviours should be addressed actively as high priority treatment targets. Early drop-out is common for BPD patients in treatment but is possible to counteract by fostering a strong therapeutic relationship through adopting a realistic, but consistent and supportive approach carefully avoiding reinforcement of suicidal behaviours...

The multistep process of hepatic carcinogenesis is mirrored by the morphologic classification of lesions detectable in cirrhosis, that include large regenerative nodules (LRN), low grade dysplastic nodules (LGDN) and high grade dysplastic nodules (HGDN). The latter belong to the "bordeline malignancy" cathegory requiring an accurate distinction from well-differentiated and early hepatocellular carcinoma. Nodules in cirrhosis are usually detected by non-invasive imaging techniques, being the latter unable to discriminate malignant from non-malignant forms, particularly in the 1-2 cm sized group...

Obesity is defined as a condition of increased adipose tissue mass. Although it is quite clear that many health complications are associated with severe obesity, it is very difficult to measure adipose tissue and precise quantification of the amount needed to increase risk to health. The cutoff between normality and obesity is still the subject of debate. For now, the body mass index (BMI) is used as index of obesity, but the curves describing the relationship with morbidity and mortality are U or J-shapes...

BACKGROUND: Leprosy is characterized by a disease spectrum having two polar clinical forms dependent on the presence or not of cell-mediated immunity. In the tuberculoid forms, granuloma-activated macrophages kill Mycobacterium leprae in conjunction with a Th1 response while, in multibacillary (MB) lesions, M. leprae nonactivated macrophages infiltrate the nerves and internal organs together with a Th2 response. The functional properties and activation pathways of macrophages isolated from patients with MB leprosy remain only partially understood...

Bronchial hyperresponsiveness (BHR) is a common feature in the majority of asthmatic subjects and methacholine is the most frequently used agent for the test. The influence of 3 or 5 min time intervals between doses steps in a double methacholine challenge test (MCH-3' or MCH-5') was investigated. Using the MCH-3' challenge, 52 intermittent asthmatics were classified as having moderate (BHR-M; 18 subjects), mild (BHR-m; 19 subjects), or bordeline (BHR-B; 15 subjects) BHR. The cumulative dose and the PD20FEV(1) were higher for MCH-5' compared with MCH-3' in BHR-m (p < 0...

INTRODUCTION: Borderline Personality Disorder (BPD) is the most studied Axis II disorders. However, there are no Spanish versions of specific interviews. The Diagnostic Interview for Borderlines-Revised (DIB-R) is a semistructured interview used to determine the diagnosis and severity of BPD patients. The aim of this study was to validate the DIB-R for use in a Spanish-speaking sample. METHOD: The psychometric characteristics of the DIB-R Spanish version were assessed in a sample of 156 patients with the possible diagnosis of borderline personality disorder...

Pelvic sonography is the gold standard for diagnosis of ovarian masses. Only 2% of adnexal masses or malignant or bordeline tumors. Does Doppler US improve the diagnostic accuracy of sonography? The purpose of this article is to review current data and identify what areas still require further evaluation with regards to Doppler characterization of adnexal masses. It has been shown that Doppler evaluation of the ovary and cyst or tumor improves the diagnostic accuracy of US to differentiate between benign and malignant lesions...

Tumor markers have been investigated in differentiation of benign and malignant tumors. We analyzed CA 125 and vascular endothelial growth factor (VEGF) levels in serum and cyst fluid in patients with epithelial ovarian tumors. Serum and tumor cyst fluid of 50 patients with ovarian epithelial tumors (7 malignant, 3 bordeline and 40 benign) were assayed for VEGF by ELISA and CA 125 levels by chemoluminescence. CA 125 serum levels were significantly higher in patients with malignant and borderline tumors than in patients with benign cysts (p = 0...
