D2 lymphadenectomy is currently the worldwide standard operation for locally advanced gastric cancer. There were no major modifications for extent of standard lymphadenectomy in Japanese gastric cancer treatment guidelines 2014(ver.4). Prophylactic para-arotic lymphadenectomy is not recommended. Although there is no high level evidence, D2+No14v may be beneficial to patients who are suspected to harbor metastasis to No6 node, and D2+No13 lymphadenectomy may be an option in a potentially curative gastrectomy for tumors invading the duodenum...

BACKGROUND: Here we investigated Brahma-related gene 1 (BRG1) expression in aortic smooth muscle cells (SMCs) and its role in the regulation of the pathological changes in aortic SMCs of thoracic arotic dissection (TAD). METHODS: BRG1, matrix metalloproteinase 2 (MMP2), and MMP9 mRNA and protein expression in human aortic specimens were examined by qPCR and western blot, respectively. The percentage of apoptotic and contractile SMCs in aortic specimens were determined by TUNEL assay and α-SMA immunohistochemical staining, respectively...

Hypotension and shock can be classified as hypotension caused by reduced or maintained left ventricular (LV) ejection. Reduced left ventricular ejection can result from intrinsic left ventricular, aortic valve or mitral valve failure, which includes dilated or ischemic cardiomyopathy, left main trunk disease, acute myocarditis, etc. Acute and subacute severe aortic regurgitation can also cause shock. Echocardiography allows noninvasive diagnosis of infective endocarditis and Takayasu's arteritis to cause severe arotic regurgitation and can also be used to diagnose obstruction of the left ventricular outflow tract...

PURPOSE: Evaluation of the diagnostic, prognostic and possible therapeutic role of extended lymphadenectomy to lower para-aortic area in operable bladder cancer patients. PATIENTS AND METHODS: One hundred and nine patients were subjected to the procedure in the National Cancer Institute of Cairo University, and in Minea Oncology Center, Ministry of Health by the same group of surgeons, during the period from September 2000 to March 2003. The lymph nodes dissected were labeled to the following groups: perivesical, lymph node of Cloquet, external iliac, internal iliac and obturator, common iliac and paraaortic groups both right and left...

The diagnostic value of two-dimensional transesophageal echocardiography (TEE) was evaluated in 29 patients with dissecting aortic aneurysm (DeBakey type I; 4, type II; 1, type IIIa; 6, type IIIb; 18) who were studied consecutively from May 1987 to August 1988. An intimal flap was demonstrated in all 29 cases. DeBakey type classification could be recognized in 26 of 29 patients (89.7%). The entry of aortic dissection could be identified in 26 of 29 patients (89.7%). The differentiation between true and false lumen could be performed in all 29 cases...

A dissecting aneurysm of the ascending and transverse aorta with heart failure due to sudden severe arotic valve insufficiency in a 47-year-old man has been treated successfully. Vasodilator therapy was done preoperatively, and then hemodynamics was improved markedly. A selective perfusion technique by using separate pumps at pre-determined flow-rates with hypothermia was utilized. The replacement of both the aortic valve and the ascending and transverse aorta involving brachiocephalic artery with a prosthetic valve and expanded polytetra-fluoroethylene coated woven Dacron grafts was performed...
