W Müller-Forell
Intracranial pathologies involving the visual pathway are manifold. Aligning to anatomy, the most frequent and/or most important extrinsic and intrinsic intracranial lesions are presented. Clinical symptoms and imaging characteristics of lesions of the sellar region are demonstrated in different imaging modalities. The extrinsic lesions mainly consist of pituitary adenomas, meningeomas, craniopharyngeomas and chordomas. In (asymptomatic and symptomatic) aneurysms, different neurological symptoms depend on the location of aneurysms of the circle of Willis...

BACKGROUND: The fast growing internet offers easy access to medical information. So far there are limited data concerning the quality of this information. This study examined quality and readability of paediatric neuro-oncological information on the internet in german language. METHOD: Using the search terms "medulloblastoma", "ependymoma", "craniopharyngeoma", "brainstem glioma" and "low grade astrocytoma" in six different search engines, the first 30 universal/uniform resource locators (URLs) of each search engine were assessed...

We report the case of a 46-year-old woman who underwent surgery for an adamantinous craniopharyngeoma (WHO grade I). The postoperative course, during which the patient received 16 mg/day of dexamethasone, was initially uneventful. After a fortnight the patient developed infectious signs and an intracranial abscess at the operation site with simultaneous purulent coxitis. Both the intracranial abscess and the coxitis were evacuated and drained. In tissue samples and pus obtained during re-craniotomy and during surgery on the hip, Salmonella enteritidis was detected by cultivation...

Most space occupying lesions of the sellar region are tumors (e.g., pituitary adenomas, craniopharyngiomas, but also meningiomas). In approximately 8% of patients with sellar mass lesions, non-neoplastic disorders (e.g., inflammations, cysts, hyperplasia) are causal. Cystic lesions of the pituitary region are classified as Rathke's cysts, colloid cysts, arachnoidal cysts and epidermoid cysts. These have to be differentiated from cystic craniopharyngeomas or cystic pituitary adenomas. Autochthonous inflammations (lymphocyctic hypophysitis, idiopathic granulomatous hypophysitis, xanthomatous and peritumorous hypophysitis) have to be distinguished from generalized diseases involving the pituitary (e...

AIM: Hormonal deficiencies, visual disturbances, or cerebral symptoms are often the symptoms through which craniopharyngiomas (CP) first become clinically manifest. As these symptoms can exist for a long period of time various specialists are often consulted before the final diagnosis is made. PATIENTS AND METHODS: Between the years 1970 and 1998, 37 children and adolescents with CP were treated in Erlangen. The documents of 35 patients (22 male, 13 female) were evaluated retrospectively...

OBJECTIVES: Space-occupying cystic lesions may develop in a variable time after resection of particular intracranial tumors, representing a small amount of complications of these procedures. We present our experience with the development and operative treatment of such postoperative cystic lesions in order to identify possible risk factors and to optimize the treatment. METHODS: The records and neuroradiological findings of patients, operated on either gliomas and meningeomas or craniopharyngeomas, who developed symptomatic cystic lesions in the former tumor resection area during the last ten years, were analyzed...

A variety of mass lesions are placed in and around of the sella turcica. Although their common location these lesions have distinguishing features and different approaches can be selected. There are conventional transcranial and sophisticated skull base approaches. We report our experience with a subfrontal route to the sellar region. Between February 1997 and March 1998, 19 tumors placed around the sella have been treated surgically by this method. Eleven of them were pituitary tumors, 3 were craniopharyngeomas, 2 were meningiomas, and 1 was germinoma...

Over the last decade, the use of rigid endoscopes in surgery of tumorous lesions of the nose, paranasal sinuses, nasopharynx and anterior skull base has extended and diversified. Endoscopic surgical approaches for malignant lesions are very controversially discussed as of today, yet. From 1989-1999 we have treated 43 patients with invasive/destructive tumors of the paranasal sinuses and the anterior skull base strictly endoscopically, transnasally. These included 5 patients with juvenile angiofibromas and 36 patients with various malignant tumors, one with a large invasive macroadenoma of the pituitary and one case of a craniopharyngeoma...

Intracavitary irradiation of intrasellar cystic craniopharyngeomas by stereotactic cyst puncture and injection of radioactive solutions is not yet possible. Therefore we designed a new method which allows such intracavitary irradiation. Its principle is a transphenoidal approach with only a small bony opening of the sella floor, followed by cyst puncture, exclusion of cyst leakage by Metrizamid injection under x-ray control, injection of Y-90-colloid solution at a dosage which delivers a radiation of 200 Gy to the cyst wall, and finally tight closure of the puncture site using fibrin glue and gelfoam...

Stereotactic endoscopic techniques are extremely helpful in diagnosis and therapy of cystic intracerebral space occupying lesions. Acute space occupying lesions can be managed effectively and without major tissue traumatization. Up to now we have operated on more than 70 cystic intracerebral space occupying lesions with a stereotactic endoscopic technique. The main diagnoses were colloid cysts, cystic craniopharyngeoma, arachnoidal and pineal cysts. In must be stressed that in cystic anaplastic astrocytomas and glioblastomas as well as metastases only an acute inner cerebral decompression can be achieved by neuroendoscopic techniques in combination with the application of reservoir systems...

Seven patients did exhibit ovulatory cycles when GnRH was administered i.v. at a dose of 20 micrograms/pulse, but not at lower doses or when GnRH was administered s.c. Pulsatile GnRH administration may be used to assess the functional integrity of pituitary gonadotrophs and to distinguish pituitary from hypothalamic site of lesion resulting in hypogonadotrophic hypogonadism...

The frequency of growth hormone (GH) deficiency in patients operated for pituitary neoplasms of various size and type was investigated using the insulin tolerance test. 45 patients were included in the study. 20 of the patients had a non-hormone secreting pituitary neoplasm, 9 had GH-, 6 ACTH-, 7 prolactin secreting adenomas and 3 had a craniopharyngeoma. Complete endocrinological examination was obtained in all patients after pituitary surgery. Apart from patients operated for GH secreting adenomas, GH deficiency was very common after pituitary surgery (92%), even in patients operated for small lesions...

A reliable bedside test for screening of visual field defects is a valuable tool in the examination of patients with a putative disease affecting the sensory visual pathways. Conventional methods such as Donders' confrontation method, counting fingers in the visual field periphery, of two-hand confrontation are not sufficiently sensitive to detect minor but nevertheless serious visual field defects. More sensitive methods requiring only simple tools are also described. In this study, a test card with four red squares surrounding a fixation target, a black dot, with a total test area of about 11 x 12...

Stereotactic cysto-ventricular shunting in three patients with congenital (subependymal cyst of the 3rd ventricle, subependymal cyst of the foramen of Monro, cyst of cavum septi pellucidi) and in a female patient with a large cystic suprasellar craniopharyngeoma is dealt with in this paper. The first operation was performed in May 1992 and the latest, being considered in this paper, in October 1993. All patients were admitted to our hospital suffering from signs of increased intracranial pressure. CT-scans revealed on the one hand an obstructive hydrocephalus subjected to the cystic arachnoid lesions, on the other hand a large hypodense suprasellar cystic tumor...

Experience gained with the operation of 108 intra- and suprasellar tumours and the results of the surgical intervention are presented. The transcranial -- subfrontal approach of hypophyseal tumours offers today a circumspect and, in a considerable part of the cases, radical solution. Intratracheal anaesthesia eliminates the surgical risk and radiotherapy prevents relapse. The situation is not quite as unambiguous in the case of craniopharyngeomas and optic tumours. Of the rare varieties, ependymoma and angioblastoma, of the frequent varieties tuberculum sellae meningioma and dermoids, have a promising prospect...
