M Gaddas, N Chaouache, M Ajina, Z Tabka, M Zaouali Ajina
OBJECTIVES: The aim of this study was to evaluate the impact of initial androgenic statute of women with polycystic ovary syndrome (SOPK) on the expected results of in vitro fertilization (FIV). MATERIALS AND METHODS: It is about an exploratory study carried out in 61 unfertile SOPK patients, having profited all from a cycle of FIV/ICSI (intracytoplasmic sperm injection). We have divided our population in two groups according to their hormonal androgenic statute: groups with hyperandrogenism and without hyperandrogenism...

In vitro fertilization without stimulation, in natural or semi-natural cycles ("FIV-NAT") offers an interesting alternative to controlled ovarian hyperstimulation (COH) for some patients. This protocol may be proposed to patients suffering from altered ovarian status (AOS) with encouraging results. The second indication might be patients presenting repeated implantation failure (IMF). FIV-NAT allows avoiding possible adverse effects of HOC on the endometrium. Results are respectively 15.4 and 16.6% clinical pregnancy rate per oocyte retrieval for AOS and IMF, and 25...

During in vitro fertilization (IVF) GnRH antagonists or agonists are used to optimize the control of organization of oocyte collection. The choice of stimulation protocol also takes into account the ovulatory, clinical, biological and ultrasound characteristics of the FIV candidate, as well as any stimulation measures already taken. Age appears to be the major predictive factor of oocyte response to stimulation and ultrasonography on the 3rd day is a predictive element of the success of oocyte collection. Determination of hormonal parameters on the 3rd day of the cycle (FSH, oestradiol, inhibin B, anti-mullerian hormone) makes it possible to refine the choice of therapy...

Thirty five follicular fluid (FF) non hematic samples were analyzed. It were obtained from patients who receive ovulation induction for in vitro fertilization (FIV) and gamete intrafallopian tube transfer (GIFT). The purpose of this retrospective study was to localize one or more markers for the oocyte maturity. The degree of expansion of the oocyte-cumulus-corona radiata complex (COCC), was measured and correlated with the absorbance of the yellow pigment from FF. The scanning spectrophotometry was made in the visible range at 350 to 600 nm...

The development of GnRH analogues enabling a hypophyseal desensitization with suppression of the gonadotrophic activity has opened new therapeutic possibilities in early puberty of central origin, metastatic prostate and breast cancers, endometriosis, fibromyomas and the syndrome of polycystic ovaries. This retrospective study concerning 93 cycles of in vitro fertilization between March and June 1987, compares the results of the combination of a GnRH agonist (busereline 0.3 mg s.c. x 2 days) with HMGs according to a "long protocol" or a "short protocol"...
