UNLABELLED: Cholangiocarcinomas have been often met in daily practice. Biliar tract neoplasmas are the most important group in adenomas and papillomas. There is the medical and social problem with cancer patients because they call doctor too late when cancer changes reach a high level and only paliative procedures are recommended. In the most cases the prothesis implanted by ERCP and surgical digestive bypasses are applied. AIM OF THE STUDY: was to evaluate some therapeutic methods in paliative treatment applied in patients with biliary tract cancer...

Gastric carcinoma is a malignancy, frequently manifesting itself in late stages, with metastatic spread into lymph nodes. Resection is the only curative therapy. A total of 149 gastric carcinoma patients were operated in the Ist Surgical Clinic during the studied five- year period. The pre-operative disease staging included the PET-CT examination, as a standard. Total gastrectomy (GE) was completed in 109 subjects, while 12 subjects underwent subtotal GE. The both procedures included D2 lymphadenectomy. In selected cases, the surgery included upper polar resection (7 subjects)...

The authors present their experience with endoscopic introduction of stents into esophagus and cardia based on data collected from a group of 520 patients. Their study included both paliative management of stenoses in carcinomas of the esophagus and cardia, as well as management of other esophageal conditions, including fistules, perforations and various types of stenoses, including the use of novel absorbable stents.

Paliative surgery in carcinoma of the head of the pancreas have thoroughly task to carry out decompression of biliar tract, remove duodenal impassability and reduce the pain. Numerous variants for decompression of biliar tract were recommended, but stil now there is no agreement about optimal approach. As method of choise for decompression of biliar tract in patients with cancer our first alternative is making safeless, with little traumatic and easy performance anastomosis. Duodenostasis in patients with carcinoma of the head of the pancreas was determinated clinicly in 40-60% of casees...

Arrhythmogenic right ventricular dysplasia (ARVD) is an inherited cardiomyopathy characterized by ventricular arrhythmias and structural abnormalities of the right ventricle (RV). ARVD results from progressive replacement of right ventricular myocardium with fatty and fibrous tissue. The precise prevalence of ARVD in the United States has been estimated to be 1 in 5000 of the general population. Recent evidence has made it clear that ARVD is a disease of desmosomal dysfunction. The main management consideration concerns whether to implant an ICD...

Appropriate surgical management of spinal metastases combines maximal neural decompression with simultaneous immediate spinal column stabilization in the context of a paliative operation undertaken to improve patients' quality of life. We have used a single-stage posterolateral vertebrectomy (SPLV) for disease of the lumbar spine, combined with bilateral costotransversectomies in the thoracic spine, for these challenging cases. In this prospective cohort study of 96 consecutive patients with metastatic disease of the spinal column for we describe our surgical technique in detail, we examine our learning curve in its use and we analyze the long-term surgical and "quality of life" results in 42 patients who underwent SPLV...

It is possible to apply external radiotherapy or brachy-radiotherapy (a moulage or an intersticial application) as well as their combination. In individual cases it is possible to use chemo-radiotherapy. In the case of the localized tumor a curative radiotherapy can be used as it is more save towards this organ...
