BACKGROUND: In breast cancer patients, the expression statuses of oestrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2) are crucial in the choice of treatment. Receptor expression in metastatic lesions can differ from the primary tumour. The aim of our study was to analyse the utility of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) to obtain samples allowing the identification of ER, PR and HER2 expression in patients with mediastinal metastases of breast cancer...

BACKGROUND: Lung cancer is the major cause of cancer death worldwide. Mediastinal lymph node staging is important for pretreatment lung cancer management. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a well established method for mediastinal lymph node staging. Although EBUS-TBNA samples are much smaller than surgical lymph node biopsies, histopathological evaluation and molecular testing can successfully be performed. One step nucleic acid amplification (OSNA), which measures cytokeratin 19 (CK19) mRNA concentration, is a target marker that is gaining importance in quick detection of lymph node metastases in breast cancer and other cancers...

Diagnostic approach to a pulmonary nodule depends upon the characteristics of the nodule and its location. Despite utilizing advanced minimally invasive diagnostic techniques, the work-up might be challenging. We report the case of a 60-year-old woman with history of breast cancer that was treated with lumpectomy 11 years earlier. She presented with a pulmonary nodule located medially in the fissure between the left upper and lower lobes. Linear EBUS was chosen as a diagnostic approach and the diagnosis of metastatic breast cancer was made without any complications...

SESSION TYPE: Endobronchial UltrasoundPRESENTED ON: Monday, October 22, 2012 at 04:00 PM - 05:30 PMPURPOSE: Metastasis of breast cancer to intrathoracic lymph nodes is common and the biopsy of suspicious lesions can have important diagnostic, prognostic and therapeutic implications, in particular tumor receptor status. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a minimally invasive, efficient tool in sampling mediastinal lymphadenopathy, but its role in the management of breast cancer while appealing is unproven...

SESSION TYPE: Miscellaneous Case Report Posters IPRESENTED ON: Tuesday, October 23, 2012 at 01:30 PM - 02:30 PMINTRODUCTION: An intrathoracic goiter is an uncommon phenomenon with presentations ranging from asymptomatic to complete respiratory failure. The incidence of true primary intrathoracic thyroid is even less common, and we present a case of asymptomatic primary intrathoracic goiter two years after total thryoidectomy.CASE PRESENTATION: Our patient is a 68 year old woman with an initial presentation of viral syndrome with incidental notation of heterogeneous thoracic inlet mass on CT scan...

SESSION TYPE: Cancer Case Report Posters IPRESENTED ON: Tuesday, October 23, 2012 at 01:30 PM - 02:30 PMINTRODUCTION: Pulmonary embolism (PE) is the most common cause of filling defects of the pulmonary arteries as detected by CT angiography of the chest. We present here two cases of an unusual patho-etiology of pulmonary artery filling defects.CASE PRESENTATION: Case 1. A 63 year-old-male was evaluated for progressive dyspnea on exertion. He had a 20 pound weight loss. Auscultation was unremarkable. The initial chest X-ray showed an enlarged right hilum...

SESSION TYPE: Miscellaneous Cases IIIPRESENTED ON: Wednesday, October 24, 2012 at 11:15 AM - 12:30 PMINTRODUCTION: Trastuzumab is an IgG1 humanized monoclonal antibody which targets Human Epidermal Growth Factor Receptor 2 (HER2). Since its development in 1998, Trastuzumab has been used for treatment of HER2 positive breast cancer. Pulmonary complications associated with Trastuzumab include acute lung injury, acute pneumonitis, organizing pneumonia, infusion-related reaction, and bronchospasm. We report a case of pulmonary sarcoidosis that developed after Trastuzumab therapy...

An 86-year-old woman with a history of right breast cancer resected seven years ago had a small pulmonary nodule located in left S5. Diagnosis was made by bronchoscopy using the endobronchial ultrasonography-guided sheath (EBUS-GS) method, but a histological diagnosis was not obtained. Wedge resection was performed due to suspicion of a metastatic lesion from breast cancer based on radiological findings. The tumor was subsequently found to be malignant melanoma of the lung. An initial diagnosis of primary melanoma of the lung was made because a melanoma lesion at another site was not seen despite a detailed work up...

As an evolving technique, linear endobronchial ultrasound is becoming the first choice and standard of care not only to diagnose the malignant and benign mediastinal lesions but also to stage non-small cell lung cancer. Lung cancer is the leading cause of cancer-related mortality in both men and women. The disease causes more death compared with colorectal, breast, and prostate cancers combined in the United States. Staging of lung cancer determines the prognosis. The type of lung cancer has changed in the past few decades...

Intrathoracic lymph node enlargement is a common finding in patients with extrathoracic malignancies. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a technique that is commonly used for lung cancer diagnosis and staging but that has not been widely investigated for the diagnosis of enlarged mediastinal and lobar lymph nodes in patients with extrathoracic malignancies. We conducted a retrospective study of 117 patients with extrathoracic malignancies who underwent EBUS-TBNA for diagnosis of intrathoracic lymph node enlargement from October 2005 to December 2009 and compared the EBUS-TBNA findings with the final diagnoses...

BACKGROUND: The association between cancer and non-caseating granulomas is controversial. The aim of this study is to describe the clinical characteristics and outcome of a cohort of patients found to have non-caseating granulomas following the diagnosis of cancer. METHODS: Retrospective review of medical records. RESULTS: There were 30 patients with non-caseating granulomas following the diagnosis of cancer. There were 21 females and 14 patients were African Americans...

BACKGROUND: Granulomatous inflammation has been previously reported in association with cancer. Endobronchial ultrasound guided transbronchial needle aspiration (EBUS-TBNA) is a new minimally invasive test for investigating mediastinal lymphadenopathy. The identification of granulomatous inflammation by EBUS-TBNA and the clinical implications of such detection in a series of patients with previously treated cancer and new mediastinal lymphadenopathy has not previously been performed. METHODS: All 153 consecutive patients undergoing EBUS-TBNA in an academic cancer institution for suspected cancer in the mediastinum (mediastinal lymphadenopathy by CT imaging) were reviewed...

Infants with acute lymphoblastic leukemia (ALL) are more resistant to chemotherapeutic drugs than older children with ALL, except for Ara-C. Drug resistance mechanisms in infant ALL, however, remain unknown. Possibly, multidrug resistance (MDR) proteins like P-glycoprotein, MDR-associated protein (MRP1), lung resistance-related protein (LRP/MVP) and the breast cancer resistance protein (BCRP) play a role. Accordingly, we measured the mRNA levels of these proteins in infants (n=13) and non-infants (n=13) with ALL, using quantitative RT-PCR...
