Truma surgery today is facing a number of significant challenges that offer a stimulus for growth and evolution of tl practice. To successfully face these challenges, reexamination of the discipline, the current practice models for its providers, and the definition/scope of the specialty will be necessary. Further development and application of the cute care surgery model may represent the future direction for trauma care practitioners.

We studied 6 cases of tracheobronchial injury due to the blunt chest truma in our department. All patients were male of 19 to 60 years of age. Injured sites were main bronchus in 2, tracheobronchial portion in 2, cervical trachea and main bronchus in 1, cervical trachea in 1. In a case of cervical tracheal injury and 2 cases of tracheobronchial injury, emergent operation was performed on the day of accident. The fascio-cutaneous flaps with distal pedicle, with the new discoveries on fascial and septal circulation of the lower limbs, are a valid reconstructing method for the loss of substance localized on the third inferior of the leg and in the heel's region. In 21 patients, 18 men and 3 women, fascio-cutaneous flaps were employed for the traumatic loss of substance, just in two cases consequentially to removal of maligns lesions, all of them were omolaterals except in three cases in which cross-leg were used. The results are satisfying if we remember that just in two cases we had partial necrosis and in other two cases we had to reinstate the flap which had survived just in his deep position...

A 15 1/2-year-old girl with disseminated lupus erythematosus presented with acute flank pain and hematuria during oral anticoagulant therapy for thrombophlebitis of the lower extremities. The intravenous pyelogram demonstrated multiple filling defects of the renal collecting system interpreted as pyelocalyceal submocosal hemorrhage. This benign complication disappeared following adjustment of the anticoagulant therapy. Phacoanaphylaxis (lens-induced uveitis) is a potentially curable ocular inflammation secondary to lens truma. Recently we have seen three clinically undiagnosed enucleation specimens in our eye pathology laboratory with this entity. Our observations add an argument to the recent concept that early lens removal in cases of traumatic cataract with lens capsule rupture would ultimately lead to better visual results.

Fifty-two patients (ages 1 to 16 years) with pelvic fractures were hospitalized at an urban medical center during a 5-year period. All patients with acetabular fractures had adjacent hemipelvis fractures. None were seen to develop symptomatic acetabular dysplasia. Stable breaks in the ring (anterior ring fracture) were encountered most frequently. While many patients were discharged ambulating within a short time, over 50% had prolonged hospitalizations due to associated truma. Only one child had serious urologic trauma, demonstrating that isolated anterior ring fractures in children are not as treacherous in terms of pelvic content injury as those one might encounter in adults...
