OBJECTIVE: The enhanced recovery after surgery program (ERAS) has become the base of perioperative management in various surgical specialties. However, limited data are available for radical cystectomy. METHODS: We have analyzed 124 patients undergoing radical cystectomy. For retrospective analysis, the patients were divided into two groups:Group A (n=72) included patients whose surgery was performed before the introduction of the ERAS protocol; and Group B (n=52) included patients who were treated following the items included in the ERAS protocol...

RANDOMIZED CONTROLLED TRIAL

The objective of this work was to determine whether the increase in serum GH, IGF1, glucose and insulin levels caused by the administration of hGH modifies the normal response of cholesterol and lipoproteins to surgical aggression. A prospective, randomized, and double blind study is carried out in 28 patients operated for gallstones and diverticulitis. The control group (n = 15) was not given anything except conventional fluidtherapy in the postoperative period: the patients in the hGH (n = 13) group were also given 8 IU of hGH during the first five days after the intervention...

The pattern of intracellular aminoacids may be reproduced individually and is different for each illness. This study seeks to measure alterations in this pattern in rats after the administration of hypocaloric peripheral parenteral nutrition (HPPN), group B, for five days, by comparing them with rats subjected to conventional fluidtherapy, group A, for the same period. HPPN tolerance was good in all cases. Our results show a higher tendency to glyconeogenesis in group A, measured by a reduction in the glycogenic aminoacids especially alanine...

An application of the KBSIM (Knowledge-Based SIMulation) system to the improved design of fluid resuscitation is described. The system integrates knowledge from three domains, viz. the pathophysiology of traumatized patients represented in a quantitative biodynamic model, the heuristics of fluid resuscitation of such patients as represented in 'production rules', and some 'metaknowledge' reflected in the design of a multi-window user interface. This technique of combining numerical simulation with symbolic reasoning has obvious advantages during the design process and in training, by giving the user a possibility to evaluate his measures by direct feedback from the system...

Three observations of acute hyperuricemic renal insufficiency with conserved diuresis appeared during the course of a citostatic treatment of a lymphoblastic acute leukemia and two lymphoblastic lymphosarcomas are studied. The evolution of nephropathy was favourable in all cases by means of allopurinol and fluidtherapy.
