BACKGROUND: In the past few years, the Canary Islands received immigrants from sub-Saharan countries that arrive to the coast after long boat trips in extreme adverse conditions. The named «trench foot» is a previously described infectious entity developed after feet wounds that get infected by being in contact with water, urine and excrements in these small and crowded boats. We describe a new clinical entity, not published yet, that consists in massive edema in the extremities associating necrosis of the skin and subcutaneous tissue that characteristically respects the muscle and with a non-infectious etiology...

Background and objective The use of arthroscopy for tibial plateau fractures type I, II and III according to Schatzker classification has increased, yet its employment for tibial plateau fractures Schatzker IV, V and VI is controversial due to the potential risk of compartment syndrome, deep vein thrombosis and infection. We aimed to compare the rate of operative and postoperative complications among patients with these types of tibial plateau fractures treated with and without arthroscopy at the time of definitive reduction and osteosynthesis...

The Guide Line of the Society for Vascular Surgery now recommends endovascular repair (rEVAR) for ruptured abdominal aortic aneurysms (RAAA) when anatomical conditions are present. Abdominal compartment syndrome (ACS) can be one of the serious postoperative complications of rEVAR. ACS is usually associated with progressive development of organ dysfunctions and poor outcomes. We describe an ACS following a RAAA with hemorrhagic shock treated conservatively with a rEVAR.Decompression laparotomy were not performed because spontaneous improvement with conservative ICU treatment was effective...

The management of a patient with limb trauma begins with a rapid assessment of the lesions in order to prioritise the treatment of life-threatening lesions, primarily haemorrhage. Severe limb trauma is defined by the presence of specific severity criteria and requires the injured person to be referred to a specialised severe trauma centre. The prognosis is twofold, both vital and functional, based on the speed as well as the quality of immediate pre-hospital care, specific hospital care and the prevention of secondary complications...

Laparostomy is defined as a surgical technique in which the peritoneal cavity is opened and left open, called "open abdomen" (OA). An open abdomen presents numerous challenges for the clinician. Specific pathologies like severe intraabdominal sepsis, trauma requiring damage control, abdominal compartment syndrome, staged abdominal repair and other complex abdominal pathologies can be managed with laparostomy. Laparostomy allows abdominal re-exploration, clearing and control of abdominal fluid of the fascia, avoiding intra-abdominal hypertension (IAH), abdominal wall closure...

Iron overload pathophysiology has benefited from significant advances in the knowledge of iron metabolism and in molecular genetics. As a consequence, iron overload nosology has been revisited. The hematologist may be confronted to a number of iron overload syndromes, from genetic or acquired origin. Hemochromatoses, mostly but not exclusively related to the HFE gene, correspond to systemic iron overload of genetic origin in which iron excess is the consequence of hepcidin deficiency, hepcidin being the hormone regulating negatively plasma iron...

Rarement isolé, il peut passer inaperçu dans les 24 à 48 premières heures de la phase aiguë d'un traumatisme sévère quand les principales préoccupations médicales sont la sauvegarde des fonctions vitales...

In this review, the intention is to present the four clinical models of HIV-1-brain damage, named with the diffuse category of HIV-encephalopathy. It contains the past three decades, since the first static-infectological model of Snider WD, developed in 1983, based in a great neuropathological trial with AIDS patients. The second one, maybe the most compact of them, was the AIDS-dementia complex, published by the Navia BA group, in two notable papers published in 1986. This resulted in a consistent HIV-neuropsychiatric condition, till 1996, when HAART era begun...

BACKGROUND AND OBJECTIVES: The adverse effects of intra-abdominal hypertension are known for many years. Only recently proper attention has been given to routine intra-abdominal pressure (IAP) monitoring. There is evidence that a quarter of intensive care units (ICU) do not measure IAP, due to a lack of knowledge of its importance or difficulty in results interpretation. The aim of this study is investigate the knowledge of ICU physicians about abdominal compartimental syndrome and its management...

During the transition from acute to chronic stages of recovery after spinal cord injury (SCI), there is an evolving state of immunologic dysfunction that exacerbates the problems associated with the more clinically obvious neurologic deficits. Since injury directly affects cells embedded within the "immune privileged/specialized" milieu of the spinal cord, maladaptive or inefficient responses are likely to occur. Collectively, these responses qualify as part of the continuum of "SCI disease" and are important therapeutic targets to improve neural repair and neurological outcome...

BACKGROUND: In the last few years laparostomy has become an even more useful option for the surgeon due to the development of the experience in the critical abdominal trauma and in the severe surgical urgencies as PANE and peritonitis with the introduction of Damage Control as surgical strategy to prevent the abdominal compartimental syndrom (ACS). After a laparostomy the surgeon needs to provide a delayed fascial closure to achieve the best outcome for the patient. The aim of this paper is to illustrate the experience and the results after the introduction of a modified laparostomy technique in our surgical activity...

Analyzing a complex case and referring to the literature, the authors describe abdominal compartmental syndrome as a complication of Fournier's gangrene, stressing out the importance of an early diagnosis to perform prompt and effective treatment. The characteristic of this case is not represented only by the Fournier's gangrene rarity, but also by the appearance of an abdominal compartmental syndrome due to the gangrene extension from the scrotum to the abdominal wall and cavity through the spermatic funiculus...

Despite advances in antimicrobial therapy and supportive measures, mortality for patients with severe community-acquired pneumonia admitted to the intensive care unit remains high, especially in case of development of sepsis with its complications. So, the early detection of the severity of pneumonia is crucial to achieve an optimal monitoring and treatment of the patients. Studies of serum and lung cytokines levels in patients with pneumonia show a compartimentalized response, that rarely appears in the serum...

The purpose of this report is to describe a case of severe snake bite with envenomation by an African puff adder (Bitis arietans). Presenting symptoms warranted administration of antivenon upon admission. The patient's general condition improved. However bite-related trauma caused extensive phlyctenuar edema of the lower extremity with a high risk of compartimental syndrome due to hardening and compression. Due to the high risk for postoperative infection at our facility, aponevrotomy to relief pressure was not undertaken immediately...

Radical debridement is specially important in case of muscular attrition. Fasciotomie of the first web is systematic in case of crush injuries for avoiding compartimental syndrome. For prevention of first web retraction, one or two Kirchner wires are placed in emergency between first and second metacarpal, and later a spilt in maximal anti-position of the first ray.
