BACKGROUND: Takotsubo cardiomyopathy (TCM) is a temporary and reversible systolic abnormality of the left ventricular apical area resembling a myocardial infarction. Cardiac rupture due to TCM is a rare but fatal complication. Without cardiac surgery, 94% of patients with left ventricular free wall rupture (LVFWR) due to TCM die. Furthermore, successful surgical cases are rare. We report herein the successful treatment of multiple LVFWRs due to TCM using a sutureless repair. CASE PRESENTATION: An 80-year-old man quarreled with his daughter and had a sudden onset of chest pain...

OBJECTIVE: To describe a novel approach to robot-assisted laparoscopic total hysterectomy (RH) for endometrial cancer (EC) that minimizes cancer sell spillage and develops a stable surgical field. DESIGN: Demonstration of the multidirectional traction method with narrated video footage. SETTING: Many reports have indicated that RH for EC has the same or superior short-term results compared to conventional laparoscopic hysterectomy (LH), and the long-term prognosis is the same (1,2)...

Although postadenoidectomy bleeding is a rare complication, it is serious and potentially fatal. Postnasal packing and cautery of the adenoid bed are the most popular methods of controlling postadenoidectomy bleeding. Many hazards and complications may arise from traditional nasopharyngeal packing and nasopharyngeal cautery. In this case series, oxidized regenerated cellulose (Surgicel Nu-Knit; Ethicon; Somerville, N.J) was inserted and used as postnasal packing to control primary postadenoidectomy bleeding in 9 cases after conventional curettage adenoidectomy...

BACKGROUND: Achieving hemostasis during surgery is a common challenge across surgical specialties and procedures, including cardiovascular and peripheral vascular procedures. Although several hemostatic agents are available, they all become less effective as bleeding becomes more vigorous. Veriset™ hemostatic patch is a topical hemostatic agent that has been approved for use on solid organs and in soft tissue. The primary aim of these studies was to assess the safety of Veriset hemostatic patch during cardiovascular and peripheral vascular procedures...

OBJECTIVE: Improved health outcomes can result in economic savings for hospitals and payers. While effectiveness of topical hemostatic agents in cardiac surgery has been demonstrated, evaluations of their economic benefit are limited. This study quantifies the cost consequences to hospitals, based on clinical outcomes, from using a flowable hemostatic matrix vs non-flowable topical hemostatic agents in cardiac surgery. RESEARCH DESIGN AND METHODS: Applying clinical outcomes from a prospective randomized clinical trial, a cost consequence framework was utilized to model the economic impact of comparator groups...

BACKGROUND: In the literature, staple line leak rate is reported to be 1-6 %, and hemorrhage rate is 2-5 % in laparoscopic Roux-en-Y gastric bypass (LRYGB). Various buttress materials are available in an attempt to reduce perioperative complications. The aims of our study are to evaluate the effect of using absorbable hemostat (SNK) as buttress material and compare its usage with bovine pericardial strips (PSD). METHODS: Between January 2006 to May 2007 and from October 2007 to December 2009, a total of 1,074 patients underwent LRYGB at our institution...

Oxidized regenerated cellulose is widely used as a bioabsorbable topical hemostatic agent. Postoperative visualization of this material through routine chest imaging, such as conventional radiography, computer tomography (CT), magnetic resonance imaging as well as sonography, may prove difficult and, to our knowledge, is not described in the literature. We describe a case where the mediastinal packing with Surgicel™ Nu-Knit™ after a mitral valve repair procedure led to a delayed obstruction of the superior vena cava, necessitating a re-thoracotomy and curettage of the hemostatic material...

BACKGROUND: Current management of severe surgical or traumatic bleeding is often achieved by manual tamponade or occlusion using devices such as tourniquets or ligatures. There are some clinical scenarios where these options are either marginally effective or impractical. The present study evaluates a new combination device (Fibrin pad) consisting of biologically active components (human thrombin and fibrinogen) delivered to the targeted site by an absorbable synthetic matrix (oxidized regenerated cellulose and polyglactin 910) in a swine severe bleeding model...

Efficacy of the oxygenated regenerated cellulose Surgicel Nu-Knit as a hemostatic agent by coronary shunting operation was evaluated. 84 patients were included in the study. Surgicel was locally used in 32 of patients after sternotomy and during wound closure. Intraoperative blood loss in the group of interest was higher than in the control group: 1215,4+/-146,3 ml (14,9+/-3,1 ml/kg) vs. 1066,6+/-106,8 ml (11,5+/-3,3 ml/kg). Surgicel Nu-Knit for the sternum bleeding permitted a good hemostatic effect without wax use...

INTRODUCTION: Uterine hemorrhage is a common problem at the time of caesarean section especially in a repeat caesarean section and when the operation is performed for placenta praevia. CASE REPORT AND DISCUSSION: We present the case of a 28 year old woman undergoing a repeat emergency caesarean section for scar tenderness. She had excessive hemorrhage from the incision site that could not be controlled by traditional management including intravenous oxytocin and ergometrine, intramyometrial prostaglandin and local hemostatic sutures, but was successfully controlled with topical oxidized regenerated cellulose absorbable hemostat...

BACKGROUND: The emergence of multi-drug resistant microorganisms presents a critical problem for patients undergoing surgery. Acidic pH, which is produced by oxidized regenerated cellulose (ORC), is a broad-spectrum physiological detriment to survival of microorganisms known to cause surgical infections. The purpose of this study was to examine the antimicrobial effect of ORC against antibiotic-resistant organisms. METHODS: ORC products were challenged with ATCC reference strains and clinical isolates of methicillin-resistant Staphylococcus aureus (MRSA), methicillin-resistant Staphylococcus epidermidis (MRSE), vancomycin-resistant Enterococcus (VRE), penicillin-resistant Streptococcus pneumoniae (PRSP), and non-resistant ATCC strains of S...

RANDOMIZED CONTROLLED TRIAL

A randomised, prospective trial to evaluate Surgicel Nu-knit with Vasolene ribbon gauze and Merocel packs, respectively. Sixty patients (36 males and 24 females) undergoing bilateral nasal surgery, each having the same procedure performed on both sides, were recruited. The mean age was 49 years (range: 16-70 years). At operation, Surgicel Nu-knit was placed in one nostril, the other nostril was randomised to Vasolene gauze or Merocel. Twenty-four hours post-operatively, patients were asked to assess the discomfort experienced in either side of the nose while the packs were in position and on removal...
