INTRODUCTION: ApneaLink™ (RESMED-Munich, Germany) is a simple and inexpensive device that determines the apnea-hypopnea index. The sensitivity and specificity of the apnea-hypopnea index are 100 and 87.5%, respectively. Our hypothesis can be used to create a treatment plan using the apnea-hypopnea index for intensive care unit patients. CASE PRESENTATION: This treatment plan has been created by determining the apnea-hypopnea index of eight Caucasian patients with a variety of diagnoses...

BACKGROUND: Methamphetamine (MA) is a potent psychomotor stimulant with high abuse and addictive potential. MA is a neurotoxic drug which is widely abused by females of childbearing age, raising serious public health concerns in terms of exposure of the fetus to the drug. Neurotoxic effects of MA on adult are well known, such as dopaminergic nerve terminal degeneration and cell death in regions of brain in some doses. OBJECTIVE: In the present study, we examined effect of prenatal MA exposure on mouse fetuses...

We report an unusual case of cerebral aneurysmal subarachnoid hemorrage (SAH) with Fabry's disease. A 42-year-old woman presented with aneurysmal SAH originated from a saccular aneurysm of the right posterior communicating artery. The patient was treated by an endovascular coil embolization of aneurysm. Postoperatively the patient recovered favorably without any neurological deficit. During her admission, the patient had a sign of proteinuria in urine analysis. The pathologic findings of kidney needle biopsy implied nephrosialidosis (mucolipidosis of lysosomal stroage disease), which is consistent with a Fabry's disease...

Aneurysmal subarachnoid hemorrhage (SAH), in addition to the direct effects of the initial hemorrhage and secondary neurological complications, predisposes to medical complications. The proportion of deaths caused by non-neurological medical complications (cardiac, pulmonary, gastrointestinal, renal, hematological) equals that from neurological complications. In particular, pulmonary complications are responsible for 50% of all deaths from medical complications. Neurogenic pulmonary edema (NPE) is an increase of interstitial and alveolar fluid occurring as direct consequence of any acute central nervous system injury...

BACKGROUND AND AIM OF THE WORK: Syncope occurs frequently in childhood and adolescence. It generally is benign, but may be a sign of pathology. The purpose of this study is to determine the incidence of syncope in children and adolescents in the Parma area, analyse the frequency of syncope due to cardiac, neurological or metabolic diseases, to identify a diagnostic procedure based on careful review, clinical features, and results of investigations. METHODS: The records of 156 children (age 1 to 18 years, mean age 10...

The current guidelines for the diagnosis of adult GHD are mainly based on the statements from the GH Research Society Consensus from Port Stevens in 1997. It is stated that diagnosis of adult GHD must be shown biochemically by provocative tests within the appropriate clinical context. The insulin tolerance test (ITT) was indicated as that of choice and severe GHD defined by a GH peak lower than 3 microg/L. The need to rely on provocative tests is based on evidence that that the measurement of IGF-I as well as of IGFBP-3 levels does not distinguish between normal and GHD subjects...

Cerebral aneurysms and arteriovenous malformations (AVM) are a common cause of stroke and cerebral hemorrage. Both are often discovered when they rupture, causing subarachnoid hemorrhage (SAH). SAH-induced vasospasm is mediated by enhanced vasoconstriction due to endothelin-1 (ET-1). We investigated whether endothelial cells (ECs) obtained from aneurysm and AVM express phenotypic and genotypic alterations contributing to the development of vasospasm after SAH. We isolated ECs from human AVM and aneurysm and then confirmed their EC origin by polymerase chain reaction and immunocytochemistry with endothelial markers...

An effect of nitrogen oxide donator Nebilet and inhibitor of NO-synthase L-Name in concentration 10(-4) and 10(-5) on some biochemical indices in vitro has been studied. It was established that an incubation of tissues with nitrogen oxide donator results in an increase of nitrits' level, some intensification of LPO and activation of antioxidant defense enzymes, especially in rats with SAH. On the contrary, an incubation of cerebral homogenate with inhibitor of NO-synthase L-NAME resulted in considerable lowering of nitrits' level, significant activation of LPO...

OBJECTIVE: We looked for the patients who were treated by short leg brace (SLB) in our hospital for the effects on the treatment of stroke. METHOD: Patients admitted to our hospital for the purposes of rehabilitation of stroke and had SLB prescription from October 2001 to October 2003 were investigated. A total of 20 patients (19 men, 11 women) were investigated in this study. Average age was 60.8 +/- 8.60 years old. Diagnoses were cerebral hemorrhage (12), brain infarction (7), and subarachnoid hemorrage (2)...

INTRODUCTION: Vertebral artery (VA) dissections can involve both the extracranial and intracranial portions of the VA. Intradural extension explains the occurrence of subarachnoid hemorrage (SAH). We have studied the rate of this extension, the risk of associated SAH and the therapeutic repercussions at the acute stage. METHODS: From 1985 to 2001, 42 patients with a recent extracranial VA dissection were admitted to our department of neurology. When the diagnosis of extracranial VA dissection (involving the first, second or third segment of the VA) was established, we looked for an ipsilateral intracranial extension (involving the fourth segment of the VA and/or the basilar artery)...

BACKGROUND AND PURPOSE: Potential severity of hemorrhage often leads to treat a cerebral arteriovenous malformation. Consequences can be very various and serious. Our first purpose is to define the different types of hemorrhage. Our second purpose is to appreciate more precisely individual hemorragic risk of a cerebral arteriovenous malformation with the study of his angioarchitecture. We performed a prospective study in order to validate a logistic model and a classification previously described...

Smooth muscle cell (SMC) migration plays an important role in restenosis after angioplasty. Myosin phosphorylation is necessary for cell migration. Fasudil is an inhibitor of protein kinases, including myosin light chain kinase and Rho associated kinase, thereby inhibiting myosin phosphorylation, and it has been clinically used to prevent vasospasm following subarachnoid hemorrage. Based on these findings, we examined the anti-migrative action of fasudil. In SMC (SM-3), fasudil (1-100 microM) inhibited SMC migration in a dose-dependent manner (p < 0...

BACKGROUND: Despite post-mortem examination and autopsy, many cases of sudden infant death (SID) remain unexplained. The aim of this study was to assess usefulness of CT-scan in the Sudden Infant Death Syndrome (SIDS). POPULATION: Twenty-three cases of SIDS had a post-mortem CT-scan evaluation of skull and brain. The pictures were retrospectively reviewed by several independent radiologists who were unaware of the circumstances of death and results of autopsy. RESULTS: Aspects of pneumatocele, probably due to lumbar puncture were found in 6 cases...

The case of a patient suffering from acute lumbosciatic pain, flaccid paraplegia, bladder retention and subarachnoid hemorrage, is reported. The patient underwent operation for a large ependymoma of the filum terminale. The author underlines the rare association of a spinal cord tumour and subarachnoid hemorrage and the opportunity of a routine mielo-radiculographic examination in case of lumbo-sciatic pain requiring surgical intervention: the patient, in fact, had been operated for "lumbar disc prolapse", in an orthopaedic department, four years before...

That migraine can present with a severe headache of sudden onset mimicking that of subarachnoid hemorrhage--the so-called "complicated migraine with meningeal manifestations" of Pearce and Foster--is not readily appreciated. Over the past three years, seven patients have been referred to us with a presumable diagnosis of subarachmoid hemorrage. In each one of these patients, the clinical features (an explosive headache, relapsing in three, and a normal neurological examination) together with appropriate laboratory investigations (cerebro-spinal fluid analysis, electroencephalography, echoencephalography, brain scan, arteriography, pneumoencephalography) have ruled out the diagnosis of subarachnoid hemorrhage, as well as less common causes of explosive headaches such as ball-valve tumors of the ventricular system, intra-cerebral hemorrhage, hemorrhage into a tumor...

The age factor is analysed for the operative indication in old patients. For the authors, operative indications are depended of mental and general status, clinical grade (I or II), localization and anatomical conformation of the aneurysm. The operation is performed only if the patient is asymptomatic or three weeks after the onset of the subarachnoid hemorrage. The peroperative hypotension is prohibited.

Cerebral vasospasm, cerebral metabolism and the acid-base-balance of blood and cerebrospinal fluid of dogs were studied during and after the experimental subarachnoid hemorrhage. Subarachnoid hemorrhage was induced by infusion of fresh blood into the basal cistern through a small sraniectomy in the base of the skull. Eighty adult mongrel dogs were used. Of these a complete recording was obtained in 32 cases under the constant controlled ventilation. The samples of the blood and cerebrospinal fluid (CSF) were obtained repeatedly at the inserted 1 to 3 hours from the Polyethylene tube inserted into the cisterna magna subarachnoid space, carotid artery and internal jugular vein...
