OBJECTIVE: This study is a sub-analysis from the patient cohort of the STARTER (Sonographic Tenosynovitis Assessment in RheumaToid arthritis patiEnts in Remission) study. The aim was to evaluate differences in ultrasound-detected joint and/or tendon involvement between patients receiving therapies based on a combination of csDMARDs and bDMARDs and those who were treated with either csDMARDs or bDMARDs in monotherapy. METHODS: 427 consecutive patients with a diagnosis of rheumatoid arthritis were recruited between October 2013 and June 2014...

Invariant NKT cells (iNKT) represent a unique subset of innate lymphocytes that play a dual role and exert a pro-inflammatory function and also a tolerogenic function that is crucial to maintain T cell tolerance and prevent autoimmune diseases like Multiple Sclerosis, Type 1 Diabetes, Rheumatoid Arhritis and Systemic Lupus Erithematosus (SLE). Although a large body of evidence indicated that iNKT cells are instrumental to counter-regulate T cell-mediated autoimmune diseases, there is still some controversy on whether iNKT cells can actively induce immunosuppression and directly dampen T cell autoimmunity...

The objective was to analyse epidemiological tendencies of rheumatoid arthritis (RA) in Dalmatia County in order to identify possible spatial clusters of RA. Patient-interviewers were trained to administer telephone surveys. 197 RA patients controlled at Rheumatology and immunology department of Clinical hospital of Split were mapped to place of residence by telephone survey. Statistical evidence of clustering was determined by calculating Poisson probabilities in putative areas. Four clusters were identified; the largest one was in the region of Sinj...

Lyme disease is a multisystem disease that can affect skin, nervous system, heart and joints. Lyme arthritis can develope in about 60% of "not treated" Lyme disease patients, 10% of patients may develope chronic arthritis. Lyme arhritis symptoms (especially chronic arthritis) is similar to rheumatoid arthritis. The purpose of this study was to establish the usefulness of examinations of serum levels of matrix metalloproteinases MMP-3, MMP-9, tissue inhibitor of metalloproteinases 1 (TIMP-1), hialuronic acid (HA) and antibodies against cyclic citrullinated peptide (anti-CCP) in Lyme arthritis, rheumatoid arthritis (RA) and patients with arthritic complaints...

OBJECTIVE: Treating early active rheumatoid arthritis (RA) with disease modifying antirheumatic drug (DMARD) monotherapy achieves incomplete outcomes and intensive treatment seems preferable. As the relative benefits of combining two DMARDs, one DMARD with glucocorticoids and two DMARDs with glucocorticoids are uncertain we defined them in a factorial trial. METHODS: A 2-year randomised double-blind factorial trial in patients with RA within 2 years of diagnosis treated with methotrexate studied the benefits of added ciclosporin, 9 months intensive prednisolone or both (triple therapy)...

OBJECTIVES: To assess glucomannan and pyridoindole derivatives for possible antioxidant therapy of rheumatoid arthritis (RA) by using the model of adjuvant arthritis (AA). We evaluated the association between clinical markers of the adjuvant arthritis model used - increase of hind paw volume (HPV), changes of body mass (CBM), and tissue gamma-glutamyl transferase (GGT) activity assessed in the spleen and the joint. METHODS: AA was induced in Lewis rats by a single intradermal injection of Mycobacterium butyricum...

COMPARATIVE STUDY

Diclophenac sodium, Voltaren, has been tested against indomethacin and placebo in a four-week double-blind between-patient trial in 182 patients with rheumatoid arhritis. The ameliorating effect of Voltaren on the usual clinical parameters was comparable to that of indomethacin, while no significant changes were observed in the placebo group. Side effects during treatment with Voltaren and indomethacin appeared mainly form the gastrointestinal tract and from CNS. While side effects from the gastrointestinal tract were similar in the two groups, side effects from the CNS appeared significantly less frequent during treatment with Voltaren than during treatment with indomethacin...

Ten patients with rheumatoid arhritis treated with gold sodium thiomalate developed thrombocytopenia without bone marrow asplasia. There was no life-threatening blood loss, but petechiae, purpura, or echymoses were seen in eight patients. The serum gold levels monitored in one patient did not exceed levels seen in patients without thrombocytopenia. In three cases peripheral blood lymphocytes were cultured in the presence of gold and tritiated thymidine incorporation was significantly increased. Eight patients responded to steroid therapy, one patient to BAL and pencillamine, and one patient to vincristine...
