BACKGROUND: After traumatic brain injury (TBI), catecholamines (CAs) may be needed to maintain adequate cerebral perfusion pressure (CPP), but there are no recommended alternative vasopressor therapies. This is an interim report of the first study to test the hypothesis that arginine vasopressin (AVP) is a safe and effective alternative to CAs for the management of CPP in patients with severe TBI. Acute limb ischemia may manifest by ischemic rest pain, ischemic ulcers, or gangrene. Acute arterial occlusion can be the result of emboli from a distant source, acute thrombosis of a previously patent artery, or direct trauma to an artery. Toe necrosis resulting from norepinephrine bitartrate (Levophed; Hospira Inc, Lake Forest, Illinois) is a rare case.

The purpose of this pilot study was to identify the relationship between the ratio of the diameter/cross-sectional area of the internal jugular vein (IJV) and carotid artery and the central venous pressure (CVP). After obtaining approval from our Institutional Review Board, ultrasound images were repeatedly obtained from participants on consecutive days when a thoracic central line was in place. Increasing doses of metaraminol bitartrate (Aramine) in a continuous intravenous infusion were used to support the blood pressure in four patients. Under such treatment a state of shock developed, characterized by intense peripheral vasoconstriction, hypotension and anuria.
