OBJECTIVE: A study was undertaken to test in a subgroup reanalysis of the URICO-ICTUS trial whether uric acid is superior to placebo in improving the functional outcome in patients with acute stroke and hyperglycemia. METHODS: Patients were part of the URICO-ICTUS trial, a double-blind study that compared the administration of uric acid versus placebo in stroke patients treated with alteplase within 4.5 hours of onset. RATIONALE: Oxidative stress is a major contributor to brain damage in patients with ischaemic stroke. Uric acid (UA) is a potent endogenous antioxidant molecule. In experimental ischaemia in rats, the exogenous administration of uric acid is neuroprotective and enhances the effect of rtPA. Moreover, in acute stroke patients receiving rtPA within 3 h of stroke onset, the intravenous administration of uric acid is safe, prevents an early decline in uric acid levels and reduces an early increase in oxidative stress markers and in active matrix metalloproteinase nine levels...

We made a crystallographic analysis of 87 mixed-nucleus stones and found 4 groups of stones of this kind, which are as follow: urico-oxalic, oxalico-uric, phosphato-oxalic and oxalic-phosphatic. We did not find any mixed-nucleus, urico-phosphatic or phosphatico-uric stones and we explain this fact according to the known lithogenic, metabolic mechanisms, in which urinary pH has an important function.
